Outcomes of infants born with esophageal atresia

Aim: To evaluate experience in treating esophageal atresia (EA) and the outcomes in our department.

Material and Method: The records of 32 infants with EA treated from January 2003 to May 2013 were reviewed. Patient demographics, associated anomalies, details of management and complications were examined.

Results: Fifty-six percent of the patients were girl. Mean gestational age was 37.2 weeks, birth weight was 2385 g. Mean admitted time was 2.27. 78% of patients were referred from other centers. Eight (25%) patients had pneumonia at admission.Twenty-five (78.1%) patients had proximal EA and distal TEF, 2 (6.3%) isolated EA, 2 EA and proximal TEF, 2 H type fistula and one patient EA and proximal+distal TEF. The associated anomaly rate was 87.5% and common associated malformations were cardiac anomalies (71.9%). 25 had primary repair, 2 fistula ligation, 3 gastrostomy, 1 esophagostomy-gastrostomy-fistula ligation. Colon interposition was done to a patient with EA and proximal TEF. Delayed primary repair was performed to a patient with isolated EA. Complications included anastomotic leakage 2 (6.3%), sepsis 5 (15.6%), stricture 3 (9.4%) and gastroesophageal reflux 7 (21.9%). The mean hospital stay was 28 days.  Eight patients died (25%). According to the Waterston’s classification, survival rates were 100 % for class A, 92.9 % for B, 56.3 % for C. Spitz: gr 1 89.5%, gr 2 53.8%, gr 3 no patient. Bremen without complications: gr 1 93.8%, gr 2 75%. Bremen with complications: gr: 1 66.7%, gr 2: 20%.

Conclusion: The most important causes of mortality are cardiac abnormalities and sepsis. Our survival rates are similar to expected rates according to Waterston and Bremen classifications which include preoperative pneumonia. However, our results are lower than the rates in the current reports according to Spitz classification. Late admission and probably after feeding are important problem for pneumonia and sepsis. We consider that mortality would have decrease by improvement in the prenatal and early postnatal diagnoses.

Özofagus atrezisi ile doğan bebeklerin sonuçlarının değerlendirilmesi

Amaç: Kliniğimizde özofagus atrezisi (ÖA) nedeniyle tedavi edilen hastaların sonuçlarının değerlendirilmesidir.

Gereç ve Yöntem: Ocak 2003 ile Mayıs 2013 yılları arasında tedavi edilen 32 bebeğin kayıtları incelendi. Demografik veriler, eşlik eden anomaliler, ameliyat ve tedavilerle ilgili detaylar ve komplikasyonlar değerlendirildi.

Bulgular: Hastaların 18 (% 56.2)’i kız idi. Ortalama doğum haftası 37.2 hafta, doğum ağırlığı 2385 g idi. Başvuru zamanları ortalama 2.27±1.36 idi. Hastaların %78’i dış merkezlerden refere edilmişti ve 8 (%25)’inde başvuruda pnömoni vardı. 25 (% 78.1)’inde ÖA ve distal TÖF, 2 (% 6.3)’sinde izole ÖA, 2’sinde ÖA ve proksimal TÖF, 2’sinde izole TÖF ve bir hastada proksimal-distal TÖF vardı. Ek anomali 28 (% 87.5)’inde olup en sık kardiyak (% 71.9) anomali gözlendi. 25 primer onarım, 2 fistül ligasyonu, 3 gastrostomi, bir özofagostomi-gastrostomi- fistül ligasyonu yapılmıştır. ÖA ve proksimal TÖF olan bir hastaya koloözofagoplasti, izole ÖA olan ve gastrostomi açılan iki hastanın birine geç primer onarım uygulandı. Diğeri başka bir merkeze gitti. Anastomoz kaçağı 2 (% 6.3), sepsis 5 (% 15.6), anastomoz darlığı 3 (% 9.4) ve gastroözofageal reflü 7 (% 21.9) hastada gözlenmiştir. Yatış süresi ortalama 28 gün idi. Sekiz (% 25) hasta kaybedilmiştir. Yaşam oranları Waterston sınıflamasına göre; A grubunda % 100, B grubunda % 92.9, C grubunda % 56.3 idi. Spitz: grup 1 % 89.5, grup 2 % 53.8, grup 3’de hasta yok. Bremen (komplikasyonsuz): grup 1 % 93.8, grup 2 % 75, Bremen (komplikasyonlu): grup 1 % 66.7, grup 2 % 20 idi.

Sonuç: Kardiyak anomali ve sepsis hastalarımızda en önemli mortalite nedenidir. Mortalite oranlarımız preoperatif pnömoninin dahil edildiği Waterston ve Bremen sınıflamalarında literatür ile uyumlu olmakla birlikte, Spitz’e göre düşüktür. Hastaların dış merkezlerden geç ve beslenerek geliyor olması pnömoni ve sepsis için önemli bir sorundur. Prenatal ve erken postnatal tanıların artmasının mortaliteyi azaltacağını düşünüyoruz.
