DELAYED DIAGNOSIS IN POSTERIOR URETHRAL VALVE: CASE REPORT

INTRODUCTION

Posterior urethral valve (PUV) is a congenital pathology, which can cause damage and loss of function in urinary system, characterized by infravesical obstruction. In this case report, a 12-year-old boy, who had kidney failure due to delayed diagnosis of PUV, is presented.

CASE

The patient was followed up by clinic of paediatric nephrology after he was diagnosed with recurrent urinary tract infection (UTI), hypertension and enuresis when he was 10 years old. In his ultrasonography of urinary system, bilateral grade IV hydronephrosis was observed. 2 years ago, the clean intermittent catheterization (CIC) was applied to empty bladder during a year. In patient’s DMSA, it had been reported that contributions of both kidneys to the total renal function were 99% for the right kidney and 1% for the left. The patient was consulted to our clinic because of a little problem in CIC. During cystoscopy, PUV was found and resected. In sixth month of post-operative period, patient’s enuresis was improved and no urinary tract infection was found.

DISCUSSION

The most common symptoms of PUV are recurrent urinary tract infection, palpable bladder, intermittent urination and weak urine flow. Rarely, diagnosis of PUV can be delayed until childhood, adolescence and even adulthood. Bomolaski et al. found that %10 of 47 children, who had delayed diagnosis of PUV, had final stage renal failure. Nevertheless, they stated that PUV is just not an infant pathology and performing Voiding cystourethrogram (VCUG) must be considered in boys older than 5-years with urinary tract problems (especially diurnal enuresis and urinary tract infection). Symptoms of prepubertal dysfunction usually do not change in patient who is late to consult. However, in our study, enuresis was improved after the resection.

CONCLUSION

We believe that VCUG and cystoscopy must be routinely performed in patients with recurrent UTI and dysfunction in urination because PUV can be present in later ages.

 

POSTERİOR ÜRETRAL VALV’DE GECİKMİŞ TANI: OLGU SUNUMU

GİRİŞ

Posterior üretral valf (PUV), tüm üriner sistemde hasar ve fonksiyon kaybına neden olabilen infravesikal obstrüksiyonla seyreden doğumsal bir patolojidir. Geçikmiş PUV tanısı nedeniyle böbrek fonksiyon kaybı gelişen  12 yaşındaki bir erkek hasta sunulmuştur.

OLGU

Hasta 10 yaşında iken sık idrar yolu enfeksiyonu, hipertansiyon ve enürezis nedeniyle çocuk hastalıkları nefroloji bölümünde takip edilmiş. Yapılan USG de bilateral grade 4 hidroureteronefroz olduğu ve nörojen mesane ile uyumlu görünüm saptanmış. Hastaya temiz aralıklı kateterizasyon (TAK) tedavisi yaklaşık bir yıl boyunca uygulanmış. Hastanın yapılan DMSA tetkikinde her iki böbreğin total böbrek fonksiyonlarına katılım oranları;  sağ böbrek % 99, sol böbrek % 1 olarak rapor edilmiştir. Hasta TAK uygulaması esnasında karşılaşılan güçlük nedeniyle tarafımızca konsülte edildi. Sistoskopi esnasında hastada PUV olduğu görüldü ve rezeke edildi. Hastanın postoperatif 6 aylık takibinde enürezisin düzeldiği ve idrar yolu enfeksiyonunun gelişmediği görüldü.

TARTIŞMA

PUV un başlıca bulguları; sık idrar yolu enfeksiyonu,  palpe edilebilen mesane, aralıklı veya zayıf akımla idrar yapmadır. Nadiren ço­cukluk çağı, adolesan, hatta erişkin çağa kadar tanı gecikebilir. Bomolaski ve arkadaşları gecikmiş tanı konulan PUV'lu 47 çocuğun %10'unda son dönem böbrek yetmezliği geliştiğini saptamışlar, PUV'un sadece infant hastalığı olmadığını, beş yaşın üstündeki erkeklerden işeme şikâyetleri, özellikle diürnal enürezis ya da idrar yolu enfeksiyonu olanlarda voiding sistoüretro grafi (VSUG) nin düşünülmesi gerektiğini belirtmişlerdir. Geç başvuran hastalarda prepubertal dönemde disfonksiyon bulgularında değişiklik genelde olmamaktadır. Fakat bizim olgumuzda enürezis şikayeti rezeksiyon  sonrası düzeldi.

SONUÇ

Tekrarlayan üriner sistem enfeksiyonu, işeme disfonksiyonu olan hastalarda ileri yaşlarda da PUV görülebileceğinden VSUG ve sistoskopi rutin olarak yapılması gerektiğine inanıyoruz.
