Pros and Cons of Cyanoacrylate Use in Circumcisions under Local Anesthesia

Introduction:

Although debates continue regarding the medical benefits, circumcision remains to be one of the most frequently performed operations in male children. We herein report the pros and cons of the use of n-butyl cyanoacrylate compared with the suturing technique in local circumcision.

Patients and Methods:

In August 2012, 236 children aged between 10 months to 13 years were circumcised under local anesthesia by the same surgeon. Since the parents of patients were persistent for the anesthesia type, all circumcisions were performed under local anesthesia. We randomly (through a shuffled list of names) selected children to form two similar groups for circumcision with cyanoacrylate (Li) and suture (Sq).

Findings: There was no statistical significance in mean age, infection, dehiscence, bleeding and need for extra dose of analgesics in both groups. In group Li, only six patients were converted from gluing to suturing because of the persistent failure of adherence and this was not statistically significant. Wound closure times were significantly lower in  Li. The cosmesis interpreted by parents at the 1st and 6th month follow-up revealed that Li was statistically superior to the Sq group in means of parents’ satisfaction about the cosmesis. The group Li had an overall cost of 2110 USD while the suture group was 464 USD.

Discussion: 

CAs appear to be useful agents for children whom circumcised under local anesthesia alone.The procedure itself is performed and may be taught easily. We suggest that appropriate patient selection appears to be the key to success. High costs may be diminished with alternative low volume products.

 

LOKAL ANESTEZİ ALTINDA YAPILAN SÜNNETLERDE SİYANOAKRİLAT DENEYİMİMİZ

Giriş:

 Sünnet, hakkındaki tartışmalar devam etse de halen dünyada en sık yapılan operasyonlardan biridir. Sünnetin çocuk için kolay ve hızlı, iyileşme sonrasında da kozmetik açıdan iyi görünmesi gerekmektedir. Prospektif çalışmamızda, ailelerinin isteği üzerine lokal anestezi altında yaptığımız sünnetlerde kullanılan sütür ve cyanoacrylate’ı (Liquiband n-butyl cyanoacrylate) kozmetik, iyileşme ve operasyon kriterleri açısından kıyasladık.

Hasta ve Metodlar:

Ağustos 2012 de, yaşları 10 ay ile 13 arasında değişen 236 çocuğa lokal anestezi altında sünnet yapıldı. Hastalar, mukoza cilt birleştirme yöntemine göre iki eşit gruba ayrıldı: yapıştırma (Li) ve sütür (Sq). Sünnet lokal anestezi altında tek bir cerrah tarafından yapıldı. İşleme ait komplikasyonlar (enflamasyon, enfeksiyon, ayrışma, kanama), cilt-mukoza anastomoz süresi, prosedürlerin maliyeti kaydedildi ve iyileşme ve kozmetik değerlendirildi.

Bulgular: 

Yaş ortalaması, enfeksiyon, ayrışma, kanama ve ekstra doz analjezik ihtiyacı açılarından iki grup arasında istatistiksel olarak anlamlılık yoktu. Li grubunda, altı hasta dirençli bir şekilde yapışmadı ve hat sütürler ile kapatıldı ancak bu istatistiksel olarak anlamlı bulunmadı. Cilt mukoza yaklaştırma süreleri kıyaslandığında Li grubu anlamlı olarak daha düşük değerlere sahipti. Sünnet sonrası 1. ve 6. ayda veliler tarafından yorumlanan kozmetik ise ailelerin memnuniyetleri anlamında Li grubunda Sq grubuna kıyasla istatistiksel olarak daha üstün olarak saptanmıştır. Maliyet hesaplamasında; sütür grubunda toplam maliyet 464 ABD Doları iken Li grubunda toplam maliyetin  2110  ABD Doları olduğu görüldü.

Tartışma: 

Cilt yapıştırıcıları lokal anestezi altında yapılan sünnetlerde kozmetik sonuçlar ve operasyon süresini kısaltmaları ve pratiklik açılarından değerlendirildiğine iyi alternatiflerdir. Yüksek maliyet, piyasadaki düşük miktarlı ürünler ile dengelenebilir.
