İnfrequent Defecation In İnfants and Children: Is There Need For Treatment?

. İn children that breastfed the defecation may delay two days or longer, although the normal of consistency. In this study we have investigated and follow-up the complaints of children about infrequent defecation, duration of infrequent defecation and treatment requirements.

The study included 29 patients that admitted to our clinic with the complaint of the constipation for 2 days or more. The study included infants and children were only breastfed, with normal weight gain, have meconium discharge in time, no hard defecation and no abdominal distention and problems of the anal area. In other word, The study included patients with infrequency defecation that they have no extra problem. The age range of the patients were between 10 days – 8 months. The average age of the patients were 2.5 months. The duration of infrequent defecation were between 2 days- 15 days. The average of duration of infrequent defecation were 5,8 days. The duration of infrequent defecation were continued between 2 weeks – 5,5 months. The average of duration of infrequent defecation were 1,5 months. The duration of follow-up of patients has been 5,8 months in average. Meanwhile 16 patients were began to complementary foods. The older children were 16 months old during the study.

As a result we found that the average of duration of infrequent defecation was 1.5 months. After the infrequent defecation has passed and after they started to have additional nutrients, we continued follow- up the patients in order to determine the result of future period of the situation by questioned the nutritional status, weights and physical examination. Non of patients has need to medical treatment.

As a result, in this study the infrequent defecation past in all patients. The situation of infrequent defecation has not converted into real constipation in non of the patients. After all, the infrequent defecation is a temporary situation and there is no need of treatment in infants and children that well-fed, enough gaining weight, no abdominal distension and non-hard defecation

BEBEK VE ÇOCUKLARDA YALANCI KABIZLIK: TEDAVİYE GEREK VAR MI?

Anne sütü ile beslenen bebek ve çocuklarda kıvamı normal olmasına rağmen dışkılamada iki gün veya daha uzun süreli gecikme olabilmektedir. Çalışmamızda geç dışkılayan, yani yalancı kabızlığı olan çocukların şikâyetleri, takip şekilleri ve süreleri, yalancı kabızlık devam etme süresi, tedavi gereksinimleri ve ileri dönem sonuçları geriye dönük araştırılmıştır.

Çalışmaya, polikliniğimize 2 gün ve üzeri kaka yapmama şikayeti ile getirilen toplam 29 bebek ve çocuk dahil edilmiştir. Çalışmaya, sadece anne sütü alan, kilo alımı iyi olan, mekonyumunu zamanında çıkarmış, sert dışkılaması olmayan, karın şişliği ve anal bölge sorunları olmayan, özetle ek sorunu olmayan ve geç dışkılayan çocuklar dahil edildi. Başvuru anında çocukların ortalama yaşı 2,5 ay idi, en küçük çocuk 10 günlük, en büyük hasta 8 aylık idi. Çocukların kaka yapma süreleri ortalama 5.8 gün olmuştur, en kısa 2 gün, en uzun 15 gün olmuştur. Yalancı kabızlık devam etme süresi ortalama 1.5 ay olmuştur, en erken 2 haftada, en geç 5.5 ayda yalancı kabızlık durumu geçmiştir. Çocukların takip süresi ortalama 5.8 ay olmuştur, bu sırada 16 hasta ek besine geçmiştir. Çalışma yapılırken en büyük çocuk 16 aylık olmuştur. Yalancı kabızlık devam etme süresi ortalama 1.5 ay olmuştur. Takiplerinde beslenme durumları sorgulanmış, kiloları tartılmış ve fiziki muayeneleri yapılmıştır. Hiç bir hastaya medikal tedavi verme ihtiyacı olmamıştır. İleri dönem sonuçlarını belirlemek adına, çocukların yalancı kabızlık durumu geçtikten ve çocuklar ek besine başladıktan sonra da takiplerine devam edilmiştir.

Sonuç olarak, çalışmaya dahil edilen tüm çocuklarda yalancı kabızlık durumu geçmiş, bu durum hiç bir hastada gerçek kabızlığa dönüşmemiştir. Sonuçta, iyi beslenen, yeterince kilo alan, karın şişliği olmayan ve sert dışkılamayan bebek ve çocuklarda yalancı kabızlığın geçici bir durum olduğu, tedaviye gerek olmadığı sonucuna varılmıştır.
