PRIMARY RENAL SYNOVIAL SARCOMA: A RARE CASE

Introduction: Synovial sarcoma (SS) is mainly derived from soft tissues. Rarely it can be localized in other sites like lungs, pleura and kidney. Primary synovial sarcoma of the kidney is a very rare malignancy with around 40 cases reported in the literature. Of these 40 cases few are in childhood.

Case: Sixteen years old girl was admitted to with colic abdominal pain lasting for a month, weigth loss and flank pain lasting for a week Ultrasonography (US) and magnetic resonance (MR) imaging revealed a heterogeneous mass of 61x64x69 mm in diameter located in the middle and upper pole of the left kidney. The patient had undergone biopsy because the mass was closely related to main vascular structures, was densely adhered to surrounding tissues and had metastasis to bone, liver and lung. At laparatomy, multiple solid mass lesions were seen on the middle and upper pole of the left kidney with the largest 2x2,5 cm in diameter and one was excised for biopsy. The patient had to have an operation for vertebral fracture caused by metastatic tumor at T10 level. Histologically the tumor was comprising of hyalinised, epithelioid spindle cells. Immunohistochemical staining investigation of the tumor yielded negative (-) staining for CD10, CD31, CD34, CD56, desmin, pan-cytoceratin, S-100, SMA, GFAP, estrogen and progesteron receptor, WT-1 and positive (+) staining for CD99, EMA, bcl-2, vimentin. Mitotic rate of the tumor was 1/10 BBA and Ki-67 proliferation index was 5-10%. Synovial sarcoma of kidney was diagnosed. The mass was excised totally by left radical nephroureterectomy.

Conclusion: Although the SYT gene translocation which is reported to be positive in about 95% of synovial sarcomas was negative in FISH assay of our case, clinical and histological findings supported the diagnosis of synovial sarcoma of kidney.

BÖBREKTE PRİMER SİNOVYAL SARKOM; NADİR BİR OLGU

Giriş:  Sinovyal Sarkom (SS) öncellikle yumuşak dokuda görülen bir tümördür. Çok nadiren, akciğer, plevra, böbrek tutulumu görülebilir. Primer böbrek sinovyal sarkomu çok nadir görülen bir neoplazi olup, literatürde 40 civarında olgu mevcuttur. Bunların çok azı çocuktur.

Olgu: Onaltı yaşında kız hasta, bir aydır kolik tarzı,  son bir haftadır da devamlı yan ağrısı ve kilo kaybı yakınması ile başvurdu. USG ve MRI’da Sol böbrek üst ve orta polünde 61x64x69 mm boyutlarında heterojen kitle izlendi. Kitle çevre dokulara ve büyük damarlara yakın ve kemik, karaciğer, akciğer metastazları olduğu için biopsi alınmasına karar verildi. Laparatomide, sol böbrek üst ve orta polde en büyüğü 2x2.5 cm. boyutlu, çok sayıda sert kitlesel lezyon görüldü ve bu kitlelerden biopsi alındı. Hasta tanı sürecinde T10 vertebrada metastaza bağlı fraktür gelişti ve opere oldu.  Histopatalojik incelemede; hiyalinize, epiteloid ve iğsi hücreler gözlendi. İmmunohistokimyasal incelemede, CD10, CD31, CD34, CD56, Desmin, pansitokeratin, S-100, SMA, GFAP, östrojen ve progesteron reseptörü, WT-1 ile (-), CD99, EMA, bcl-2, vimentin ile (+) boyanma saptandı. Tümör mitoz oranı 1/10 BBA ve Ki-67 proliferasyon indeksi % 5-10 bulundu. Bu bulgularla sol böbrekte Sinovial Sarkom tanısı konuldu. Kemoterapi sonrası sol radikal nefroüreterektomi ile kitle total çıkarıldı.

Sonuç: Sinovial sarkomlarda %95 civarında pozitif olduğu bildirilen SYT gen translokasyonu, yapılan FISH analizinde bizim hastamızda gösterilememiş, ancak klinik ve histolojik olarak hastaya böbrekte Sinovial Sarkom tanısı konulmuştur.
