AUTOAMPUTATED APPENDIX - A CASE REPORT

Introduction / Aim: To discuss the condition of a child patient with abdominal pain related to autoamputated appendix.

Case Report: A male patient at 12 has admitted to our clinic with abdominal pain and vomiting complaints lasting for two days. His white blood cell (WBC) count was 9,100 and C-reactive protein (CRP) level was 27,89. Although direct radiographs have reflected normal structure, there was a diffuse thickening on ileum and caecum walls with peripheral fluid collection. The patient was hospitalized. His abdominal pain has temporarily relieved but, he has undergone surgical intervention as his abdominal pain has exacerbatively recurred at the end of day one. During intraoperative exploration, it was observed that the appendix was totally seperated from caecum and its mesenteric perfusion was partially deteriorated. Appendectomy operation was performed without caecal suturation. The patient was discharged from hospital two days after the operation with total remission.

Discussiion: There are only a few cases in the literature reporting absence of appendix. It is considered that the absence of appendix is associated with autoamputation. Beside this, there is no such case reported to date which exhibits appendix autoamputation during surgical intervention.

Conclusion: It is noteworthy that appendix autoamputation was intraoperatively observed in this case. Our case may contribute to more clearly understand the ethiopathogenesis of absence of appendix fact.

OTOAMPUTE APANDİSİT – OLGU SUNUMU

Giriş/amaç: Otoampute apandikse sahip karın ağrılı bir çocuk hastayı tartışmak

Olgu sunumu: 12 yaşında erkek hasta kliniğimize 2 gündür devam eden karın ağrısı, kusma şikayetleri ile başvurdu. Yapılan tetkiklerinde beyaz küresi 9.100, CRP 27.89 geldi. Direk grafisi normal sınırlarda olan hastanın tüm batın USG’sinde ileum ve çekum duvarında diffüz kalınlaşma ve çevresinde serbest sıvı izlendi. Hasta takibe alındı. Karın ağrısı bir miktar rahatlayan, ancak 1. gün sonunda ağrısı artan hasta operasyona alındı. Yapılan eksplorasyonda hastanın apendiksinin çekumdan tam kat olarak ayrı olduğu ve mezo ile beslenmesinde bir miktar dolaşım bozukluğu olduğu görüldü. Otoampute olan apendiks kökü ile çekum segmentinde defekt yoktu. Çekuma sütür konulmadan apendektomi yapıldı. Postoperatif dönemde sorunu olmayan hasta 2 gün sonra şifa ile taburcu edildi.

Tartışma: Literatürde absans apendikse sahip çok az sayıda vaka sunumu mevcuttur. Absans apendiksin, teorik olarak apendiksin otoamputasyonuna bağlı olduğu düşünülmektedir. Bununla birlikte apendiks otoamputasyonunu cerrahi eksplorasyon sırasında gösteren bir vaka sunumu yoktur.

Sonuç: Bu vakada enteresan bir şekilde; apendiks otoamputasyonu cerrahi eksplorasyon sırasında görülmüştür. Olgumuz; absans apendiksin etyopatogenezini açıklamaya yardımcı olabilecek bir olgudur.
