A Rare Seen Thoracal Mass: Lipoblastom
Aim: To present a rare pathology in thoracal masses.
Case: At the pediatric clinic; ten-month-old boy child presenting with cough and fever, a thoracal mass had seen on observing chest x-ray that's why the patient send our clinic. A mass had detected in the left upper-middle mediasten, 68x53x55 cm sizedd at the toraks BT. the mass has removed which is intraplevral, encapsulated, lipomateus and also pushes left lung's upper lobe. After surgery, 1st day patient began to feed,chylous fluid came from his chest drainage tube; that's why his feeding stopped and TPN began. on the 3st day after the surgery;feeding-with MCT formula restarted. The following days there was no leakage so the patient's drainage tube has taken out. Pathological examination revealed lipoblastom. The patient has no problem, in first month after surgery and is followed up.
Result: Although lipoblastom is a benign pathology; it can relapse very often. In case of lipamatosis it must be remembered and followed up closely.
Nadir Görülen Bir Torakal Kitle : Lipoblastom
Amaç: Çocukluk çağında torakal kitleler içinde nadir görülen bir patolojinin sunulmasıdır.
Olgu: 10 aylık erkek çocuk öksürük, ateş yakınması ile başvurduğu pediatri kliniğinde çekilen akciğer grafisinde sol hemitoraks üst alanda opasite görülerek klniğimize refere edildi.Toraks BT’de sol üst-orta mediastende 68x53x55 mm boyutunda, sol hemitoraksın yarısını dolduran, yağ dansitesinde kitle saptandı. Torakotomide ekstraplevral yerleşimli, sol akciğer apeksine yaslanan ve posterior mediastene uzanan, kapsüllü, lipomatöz kitle görülerek tamamen çıkarıldı. Postoperatif 1. günde oral beslenmeye başlanan hastanın toraks dreninden şilöz mayi gelmesi üzerine beslenmesi kesilerek TPN başlandı. Postoperatif 3. günde MCT içerikli mama ile beslenmeye başlandı. Takibinde toraks dreninden şilöz kaçağın sona ermesi üzerine postoperatif 13. günde toraks dreni alındı.
Çıkarılan materyalin patolojisi lipoblastom olarak yorumlandı. Hasta şu anda 1. ayda sorunsuz olarak takip edilmektedir.
Sonuç: Lipoblastom benign bir patoloji olmasına rağmen sıklıkla nüks edebilir. Lipomatöz karakterli kitlelerde akla getirilmeli ve cerrahi sonrasında nüks açısından yakından takip edilmelidir.
