URETERORENOSCOPIC STONE TREATMENT IN CHILDREN

Introduction and Objective: In this study, we aimed to determine the effectiveness of ureterorenoscopy (URS) in children with upper urinary tract stone disease.

Methods: A total of 203 children (254 intervention) who underwent URS stone extraction for upper urinary tract stone between 2007 and 2012 were reviewed.

Results: One hundred and fifteen were boy, 88 were girl. Mean age was 7.2 years; 39 patients were under 2 years of age, 37 were between 2 and 5 years of age, 83 were between 5 and 10 years of age, 44 were older than 10 years. Mean stone number was 1.6, while mean stone size was 9.7 (5-21) mm. Stone was located in kidney in 47 patients, proximal ureter in 14, mid ureter in 5, and distal ureter in the remaining 137 patients. Whereas rigid URS was used in 180 (88.7%) children, flexible URS was used in 23 patients. URS was performed on right urinary system in 109 patients, left system in 73, and bilaterally in 21 cases. None of the patients required open surgery. JJ-stent was placed in only 38 (18.7%) patients. Postoperative complications were residual stone in 19 patients, mild ureteral damage in 3, stone migration to lower pole calices in 2, and hypertension in one patient. ESWL required in 7 patients and re-URS in 6 patients due to residual stone.

Conclusions: URS can be safely and successfully performed in any age group of children with appropriately selected renal and all uncomplicated ureteral stone when using appropriate instruments in experienced hands. Flexible URS is appropriate, safe and successful in the treatment of proximal ureteral or lower pole renal stone in experienced hands. However, because ureteral diameter is relatively small and particularly proximal ureter is mobile and fragile in children, inappropriately selected cases, usage of inappropriate instruments or aggressive unsafe interventions by an inexperienced hand may significantly increase the morbidity rate in such children. In addition, it also increase the treatment cost by decreasing life-time of expensive URS instruments.

ÇOCUKLARDA ÜRETERORENOSKOPİK TAŞ TEDAVİSİ

Amaç: Çocuklarda üreterorenoskopi (URS) ile tedavi edilen böbrek ve üreter taşlı çocuklardaki deneyimlerimizi paylaşmak.

Gereç ve yöntem: 2007-2012 yılları arasında böbrek veya üreter taşı tanısıyla URS uygulanan 203 çocuk (254 girişim) değerlendirildi.

Bulgular: Olguların 115’i erkek ve 88’i kız idi. Ortalama yaşları 7.2 yıl iken, 39’u 2 yaşından küçük, 37’si 2-5 yaş arasında, 83’ü 5-10 yaş arasında ve 44’ü 10 yaşından büyüktü. Ortalama taş sayısı 1.6 iken taş boyutu 9.7 (5-21) mm idi. Üriner taş, olguların 47’sinde böbrek, 14’ünde proksimal üreter, 5’inde orta üreter ve 137 olguda ise distal üreter yerleşimli idi. Olguların 180’inde (%88.7) rijid URS kullanılırken, 23 olguda fleksible URS uygulandı. Olguların 109’unda sağ üriner sisteme, 73’ünde sol üriner sisteme ve 21’inde bilateral URS uygulandı. Hiçbir hastaya açık cerrahi uygulanmadı. Toplam 38 (%18.7) olguya JJ-stent yerleştirildi. Olguların 19’unda rezidü taş, 3’ünde hafif üreter yaralanması, 2’sinde taşın böbrek alt kaliksine migrasyonu ve 1 olguda hipertansiyon gelişti. Rezidü taşların 7’sine ESWL ve 6’sına da tekrar URS uygulandı.

Yorumlar: Uygun seçilmiş aletler ve yeterli deneyim ile doğru seçilmiş böbrek ve komplike olmayan (anomalisiz) üreter taşlarının tamamı URS ile güvenle ve yüksek başarı ile çıkarılabilir. Deneyimli ellerde, özellikle proksimal üreter ve küçük alt pol taşlarının tedavisinde fleksible URS uygun, güvenli ve etkindir. Ancak, çocuklarda üreter nispeten dar ve özellikle proksimal üreter mobil ve zayıf olduğu için; uygun seçilmeyen olgular, uygun olmayan enstrumanların kullanılması veya yetersiz deneyime bağlı agresif yanlış girişim yapılması çocuklarda morbiditeyi belirgin arttırırken, pahallı URS aletlerinin kullanım ömrünü de belirgin azaltarak maliyeti yükseltmektedir.
