Intestinal Perforation with Digestion of Multiple Mini Magnets

Background: Digestion of foreign bodies is usual in childhood. They are thrown 

from gastrointestinal tract 80 % without any problems. Endoscopic interference is 

needed 10-20 % of the events and only less than 1% of the events is solved by 

surgical procedures because of the situation of intestinal obstruction, perforation or 

developing fistula.

Case: 2 years old patient was admitted because of swallowing multiple mini magnets 

6 hours ago and 4 years old patient was admitted swallowing them one day ago. 

By abdominal X-Ray examination, there were fourteen and eight mini magnets in 

their stomachs. At first flexible endoscophy was performed to both of the children. 

Magnets in stomach were grabbed by grasper. Laparotomy was performed then, 

because of there were pieces buried in gastric mucosa. Three magnets have been 

removed from two years old boy, from the fundus and lower esophagus, in which 

there were perforations. Two magnets were buried in the gastric fundus and one from 

the jejunum in four years old boy, which wasburied to the stomach five centimeters 

distal of the Treitz have been removed and the perforations have been anostomosed. 

There were no complications postoperatively in both of the children.

Conclusion: Multiple magnet digestions generally needs surgical intervention, that 

is why the families are needed to be educated well and preventive measures are 

needed to be done for the public health.

Çoklu Mini Mıknatıs Yutulmasına bağlı İntestinal Perforasyon

Giriş: Çocukluk döneminde, yabancı cisimlerin yutulması sıklıkla karşılaşılan 

olaylardır. Gastrointestinal sistemden  %80 problem olmadan atılır. Olguların %10-

20’sinde endoskopik girişim gerekirken ,%1 den az olguya intestinal obstruksiyon, 

perforasyon ya da fistül gelişmesi nedeni ile cerrahi işlem gerekir.

Olgu sunumu: İki yaşındaki çocuk altı saat önce, dört yaşındaki hasta da bir gün 

önce çoklu mini mıknatıs yutma öyküsü ile getirildi. Çekilen ayakta direkt karın 

grafisinde midede sekiz ve on dört adet mıknatıs saptandı. Her iki çocuğa önce 

bükülebilir gastroözofagoskopi yapıldı. Mide içerisine uzanan mıknatıs parçaları 

yakalayıcı ile yakalandı. Mide içerisine gömük mıknatıs parçaları gözlenmesi nedeni 

ile laparotomiye geçildi. İki yaşındaki erkek hastanın mide fundusu ile alt özofagus 

içerisine gömülü ve her iki yapıyı perfore eden üç adet mıknatıs çıkarıldı. Dört 

yaşındaki erkek çocuğun mide korpus içine gömülü iki mıknatıs ve Treitzden 5 cm 

distalde yer alıp mideye yapışmış olan jejenumdan bir mıknatıs çıkarılarak perfore 

alanlar onarıldı. Ameliyat sonrası çocuklarda bir problem izlenmedi. 

Sonuç: Çoklu mıknatıs yutulmaları genellikle cerrahi işleme gerek duyduğundan 

ailelere uygun eğitim verilmeli ve toplum için koruyucu önlemlerin alınması 

gerekmektedir.
