Laparoscopic Surgery should be first choice in Meckel's diverticula
PURPOSE Meckel’s diverticulum is the most common congenital anomaly of gastrointestinal tract. It mostly remains slient throughout life or else gives symptoms when it causes complications. Aid of radiologic modalities are very limited in diagnosis. Physicians should consider Meckel’s diverticulum while dealing with acute abdomen to prevent delays in diagnosis. Lately, minimal invasive surgery is became a choice of diagnosis and treatment. Here we present our technical details and results of laparascopy assisted management. MATERIAL AND METHOD We retrospectively studied nine cases of meckel’s diverticulitis threated with laparascopy assisted surgery ,in last three years. Seven patients were male and three were female. The median age was 7.7 years(1month - 12years) The most common symptom was gastrointestinal bleeding. First 10 mm umblical port and 3 or 5 mm working side ports were inserted. Starting from cecum, ileal loops were examined until the meckel’s diverticulum was found. The diverticulum was freed and taken out of abdomen with the umblical port while it was held by a grasper from the top. Segmental resection with primary anastomosis or wedge resection was performed outside the abdomen. All the loops were thrusted back to the abdomen and port sides were stured. No complications occured throughout or afterwards the surgical proseduce. CONCLUSION Due to small size of incision and excellent results of cosmesis, laparascopy assisted management can be proposed as golden standart especially in cases colsulted for gastrointestinal bleeding or for other complications of diverticulitis. Surgeon should keep in mind that; in the cases with intestinal obstruction, where abdominal distention dominates the clinical presentation, technical difficulties may encountered. Resection and anostomosis is performed outside the intestinal cavitiy so the surgical costs are minimal and advanced laparascopic skills are not required. In cases prediagnosed with Meckel’s diverticulum, laparascopy is a safe and effective choice of modality.
