LAPAROSCOPIC APPROACH IN A CASE WITH ISOLATED TUBAL TORSION

Introduction: Isolated tubal torsion (ITT) is  a rare cause of acute abdomen in children . Early diagnosis and treatment are important due to its effects   on future fertility. Laparoscopic approach in a case with ITT is presented.

Case report: A 12-year old female patient was admitted to our hospital with a right lower quadrant pain lasting for 4 hours. Physical examination revealed left and right lower quadrant pain both and guarding. She had a history of follow-up due to a right ovarian cyst by another institute in the last year. There was a cyst with a diameter of 5,5 cm tought to be arised from right ovary in pelvic ultrasound and the ovarian blood supply was normal. An emergent diagnostic laparoscopy performed due to acute abdomen. Right tuba was twisted around itself and right ovary was normal. After introducing 5 mm working ports from right and left lower quadrants, right tuba was detorsioned and a paratubal cyst was seen and then excised. Although left ovary was normal there was also a torsioned and pedunculated cyst in left ovary and it was also excised. Postoperative course was uneventful and histopathologic examination revealed mesonefric paratubal cyst.

Conclusion: The details of surgical technique were presented and it’s shown that laparoscopic minimal invasive surgery in diagnosis and treatment of ITT due to paratubal cyst can be perform safely.

 

 

İZOLE TUBAL TORSİYONLU BİR OLGUDA LAPAROSKOPİK YAKLAŞIM

Giriş:  İzole tubal torsiyon (İTT), çocuklarda akut karının nadir bir nedenidir. Fertiliteyi etkileyebileceği için erken tanı ve tedavi önemlidir. Paratubal kiste bağlı İTT lu bir hastaya laparoskopik yaklaşım sunulmuştur.

Olgu sunumu : 13 yaşında kız hasta 4 saattir devam eden sağ alt kadran ağrısı şikayeti ile hastanemize başvurdu. Fizik muayenesinde her iki alt kadranda hassasiyet ve defans mevcuttu. Öyküsünde 1 yıldır sağ over kisti nedeniyle dış merkezde takip edildiği öğrenildi. Pelvik ultrasonografide sağ overden kaynaklandığı düşünülen  5,5 cm çapında kist izlendi ve her iki overde kan akımı görüldü. Akut karın tablosunda olan hastaya acil tanısal laparoskopi yapıldı. Laparoskopide sağ tubanın kendi etrafında üç tur döndüğü görüldü. Sağ over normal görünümdeydi. Sağ ve sol alt kadrandan 5 mm’lik çalışma portları girildi. Sağ tuba detorsiyone edildi ve  paratubal kist izlendi. Paratubal kist laparoskopik olarak eksize edildi. Sol over normal izlendi. Ancak sol tubada da saplı kist izlendi ve torsiyone olduğu görüldü, kist total eksize edildi. Hastanın postoperatif sorunu olmadı. Kistlerin histopatolojik incelemesi, mezonefrik paratubal kist olarak rapor edildi.

Sonuç: Ameliyatın teknik detayları gösterilerek paratubal kiste bağlı İTT’nin tanı ve tedavisinde  güvenle laparoskopik minimal invaziv  cerrahinin uygulanabilirliği  gösterilmiştir.
