Experience of Pilonoidal sinus in children

Introduction: Although pilonidal sinus is seen in all ages, it is known to be less common during the pre-adolescent and adolescent periods. Pilonidal sinus (PS) is a sacrococcygeal disease impairing daily activities and quality of life. In this study, we aim to present our experiences concerning the outcome of the PS cases treated in our clinic.

Material and Method: We performed a retrospective study including 25 patients with PS disease that were treated between January 2010 and June 2013.

Results: As 17 (68%) of the cases were male and 8 (32%) were female, the mean age was 16.08 ± 1.2. According to the body mass index, 17 (68%) of the cases were normal, 4 (16%) were overweight, and 4 (16%) were obese. 48% of the patients had a moderate amount of body hair, whereas 52% had high amount of body hair. 18 (72%) of the cases reported passing 4 hours of the day sitting. While 20 (80%) of the cases received total excision and primary closure, 5 (20%) received Limberg flap repair. As of now, no recurrence was determined in the follow up of the patients.

Conclusion: There is little data about PS in the literature. Obesity, an increasing problem of childhood in our country as well, is one of the important factors of PS development. A body mass index higher than normal, significant amount of hairiness, immobility, and sitting on a firm ground are considered as the predisposing factors of sinus infection. Although there are various treatment modalities, total excision and primary closure are reported to produce successful outcomes.

Çocuklarda, Plonoidal sinüs deneyimlerimiz

Giriş: Pilonidal sinüs her yaşta görülmesine karşın pre-adölesan ve adölesan dönemde daha ender olarak ortaya çıkmaktadır. Pilonidal sinüs (PS) sakrokoksigeal bölgede rastlanan, günlük aktiviteyi ve yaşam konforunu bozan bir hastalıktır. Bu çalışmada PS hastalığı nedeniyle kliniğimizde tedavi edilen olguların sonuçlarını ve deneyimlerimizi sunmayı amaçladık.

 

Hastalar ve Yöntem: Kliniğimizde 2010 ocak – 2013 haziran tarihleri arasında, PS hastalığı nedeniyle tedavi edilen 25 olgunun verileri geriye dönük olarak incelenmiştir.

 

Bulgular: Olguların 17’si (%68) erkek, 8’i (%32) kız ve yaş ortalaması 16.08±1.2 dır. Vücut kilo indeksine göre olguların 17’si (%68) normal, 4’ü (%16) kilolu, 4’ü (%16) aşırı kiloluydu. Hastaların %48’i orta, %52’si ileri derecede kıllı cilde sahipti. Olguların 18’de(%72) günde 4 saat’den fazla oturma mevcuttu. Olguların 20’sine (%80) total eksizyon ve primer kapama, 5’ine (%20) limberg flepli onarım yapıldı. Olguların bugüne kadarki takip muayenelerinde nüks saptanmadı.

 

Sonuç: Literatürde çocukluk döneminde PS’e ilişkin çok az veri bulunmaktadır. Ülkemizde de çocukluk döneminde giderek artan bir sorun olan obezite PS gelişimi için önemli etkenlerden birisidir. Vücut ağırlığının normalden fazla olması, kıllanmanın belirgin olması, hareketsizlik ve sert zeminde uzun süre oturma sinüs enfeksiyonu için hazırlayıcı nedenlerdir. Tedavide çeşitli yöntemler olmakla birlikte total eksizyon ve primer kapama seçilmiş olgularda iyi sonuçlar vermektedir.
