A MAGNET IS NOT AN ORDINARY FOREIGN BODY

Introduction: Foreign body ingestion is common in children. They were mostly excreted from the gastrointestinal tract spontaneously. However, especially multiple magnets may lead to obstruction, fistula formation and perforation in the gastrointestinal tract. In this study, we  evaluated the patients admitted for the complaint of swallowing magnets.

Materials and Methods: Patient records between 2008-2013 were evaluated retrospectively.in terms of age, gender, the number and location of swallowed magnet and the treatment method.

Results: There were a total of 6 patients who swallowed magnets. The mean age was 4.5 ± 2.5 years. 3 patients (50%) were female and 3 (50%) were male. One patient had symptoms of obstruction. Endoscopic evaluation was performed in 5 patients and one patient underwent open surgery directly. The magnet was removed with flexible endoscope in one patient.  In a patient that swallowed multiple magnets, one magnet could be removed with the endoscope, while the other two are buried in the mucosa of the stomach and could not be extracted. In one patient, endoscopy was performed to make sure about the magnet location and number. Magnet could not be removed due to the adherence to the instruments of endoscope and was left to spontaneous discharge because of being single. In a total of 4 patients, the magnet was removed  with open surgery. In one case, intestinal closed perforation was detected during the operation. Clinical data is located in the Table.

Conclusion: Magnets usually leaves the gastrointestinal tract without any problem when single magnet with small size is swallowed. However, when multiple magnets are swallowed, because of compressing the stomach or intestinal wall, patients are at high risk of obstruction, perforation or fistula formation. One obstruction and one perforation are seen in these series. There may be some difficulties due to the adherence of magnets together and  buried into the mucosa and the adherence to endoscopic instruments. However, it is appropriate to perform before open surgery due to being minimally invasive. Early intervention is essential for the patients who swallowed multipl magnets considering the complication risks.

Giriş:

Yabancı cisim yutulması çocuklarda sıktır. Bunlar çoğunlukla gastrointestinal sistemi (GİS) kendiliğinden terk eder. Fakat özellikle birden fazla sayıda olan mıknatıslar, GİS’ de obstruksiyon, fistülizasyon ve perforasyona yol açabilir. Bu çalışmada mıknatıs yutma yakınması nedeniyle başvuran hastalar irdelendi.

Gereç ve Yöntem:

2008-2013 yıllarına ait hasta kayıtları yaş, cinsiyet, yutulan mıknatıs sayısı, yerleşim yeri ve uygulanan tedavi yöntemi açısından geriye dönük olarak incelendi.

Bulgular:

Mıknatıs yutan toplam 6 hasta vardı. Ortalama yaşları 4,5 ± 2,5 yıldı. Olguların 3’ü (% 50) kız, 3’ü (% 50) erkekti. Başvuruda bir hastada obstrüksiyon bulguları vardı. Hastaların 5’ine endoskopik değerlendirme yapıldı; bir hastaya doğrudan açık cerrahi uygulandı. Bir hastada bükülebilir endoskop ile mıknatıs çıkartıldı. Çoklu mıknatıs yutan bir hastada mıknatısların biri endoskop ile çıkarılabildi. Diğer ikisi mide mukozasına gömülüydü; çıkartılamadı. Bir hastada mıknatısın yerleşimi ve sayısından emin olmak için endoskopi yapıldı. Endoskopun çalışma enstrümanlarına yapıştığı için çıkarılamadı ve tek mıknatıs olması nedeniyle spontan deşarja bırakıldı. Toplamda 4 hastada açık cerrahi girişim ile mıknatıs çıkartıldı. Ameliyatta bir olguda intestinal kapalı perforasyon saptandı. Klinik veriler Tablo’da yer almaktadır.

Sonuç: Mıknatıs tek yutulduğunda eğer boyutu da küçükse GİS’i genellikle sorunsuz terk eder. Fakat birden fazla sayıda mıknatıs yutulduğunda araya mide veya barsak duvarı sıkışabileceğinden obstrüksiyon, perforasyon ve fistülizasyon riski yüksektir. Seride de bir obstrüksüyon ve bir perforasyon görülmüştür. Mıknatıslar birbirlerine yapışarak mukoza içine gömüldüğü ve kullanılan endoskopik enstrümanlara yapıştığı için zorluklar yaşanabilmektedir. Bununla birlikte minimal invaziv olması nedeniyle, açık cerrahiden önce denenmesi uygundur. Komplikasyon riskini düşünerek birden fazla mıknatıs yutan hastalara erken müdahale edilmesi gerekmektedir.
