CROSSED ECTOPIC TESTES WITH UNUSUAL CLINICAL FINDINGS: REPORT OF FOUR CASES

Crossed ectopic testis is an extremely rare abnormality, characterized by migration of one testis towards the opposite inguinal canal, usually associated with unilateral inguinal hernia. Here, four cases of crossed ectopic testes with unusual clinical histories were presented.

Cases

Case 1: A five years old boy admitted to our clinic with bilateral undescended testes. On physical examination, both testes were palpated in the right groin region and US supported this finding. Transverse incision of the right inguinal region visualized both testes at the level of external inguinal ring. After hernia sac was prepared and tied, both testes brought down sequentially into ipsilateral and contralateral hemiscrotums with transseptal incision.

Case 2: A six years old boy presented with bilateral undescended testes. On physical examination, left testis could be palpated at inguinal region, but right testis was impalpable and also it was not visualized by US. At surgery, a transverse incision of the left inguinal region was performed and left testis was found at the level of external inguinal ring. During the preparation of the hernia sac, right testis was found in the sac. Similarly, both testes were brought down to both hemiscrotums with transseptal incision.

Case 3: An eight years old boy presented with left impalpable testis and right inguinal bulging. On physical examination, right testis was palpated in right inguinal swollen region. Left testis was neither palpated nor visualized by US. At surgery, transverse incision of the right inguinal was made and right testis was found at the level of external inguinal ring and left testis was found in the hernia sac. In addition, ductus deferens of the testes were fused proximally. After sac was prepared and tied, same surgical technique was successfully performed.

Case 4: A two years old boy admitted to clinic with operated left inguinal hernia and bilateral nonpalpable testes. On physical examination, both testes were nonpalpable, scrotum was hypoplastic and there was left inguinal transverse inguinal scar. Bilateral testes were visualized in the right inguinal canal entrance on the ultrasound. During laparoscopy, both testes were visualized near internal inguinal canal entrance in the abdomen. At operation, right transverse incision was made and both testes were founded. There was no cord abnormality or remaining mullerian structures.  Multiple cystic appearance was present on the surface of testes, so that testis biopsy was performed.  After hernia sac was tied, bilateral transseptal orschidopexy was done. Histological examination revealed testis tissue.

In conclusion, four cases reported here have unusual clinical histories for crossed ectopic testes, such as bilateral undescended testes with palpable or nonpalpable testes, inguinal swelling instead of hernia and operated inguinal hernia. But same surgical technique was performed to all and impalpable testes were found in hernia sac. Fusion of ductus deferens was determined in one case but accompanying mullerian duct anomalies were not present. We think the diagnosis of crossed ectopic testes should not be overlooked in undescended testis cases with unusual clinical findings. 

ALIŞILMADIK “CROSSED ECTOPİC TESTES” KLİNİK BULGULARI İLE BAŞVURAN DÖRT HASTA

 “Crossed ectopic testis” nadir görülen bir anomalidir. Testisin karşı inguinal kanala göçü ile ilişkili olup genellikle tek taraflı inguinal herni ile birlikte görülür. Burada, “crossed ectopic testis” olan ve alışılmadık klinik öyküyle veya bulgularla başvuran 4 olgumuzu sunmayı amaçladık.

Olgu 1: 5 yaşındaki hasta kliniğimize iki taraflı inmemiş testis nedeniyle başvurdu. Fizik muayenede, her iki testis kasıkta sağ tarafta palpe edildi. Ultrason muayene bulgularını doğruladı. Operasyonda sağ inguinal transvers insizyon yapıldı ve her iki testis eksternal inguinal ring hizasında bulundu. Herni kesesi bağlandıktan sonra her iki testis eş zamanlı olarak transseptal insizyon sonrası skrotuma yerleştirildi.

0lgu 2: Altı yaşında hasta iki taraflı inmemiş testis bulgularıyla kliniğimize başvurdu. Fizik muayenede sol testis inguinal bölgede palpe edildi, sağ testis palpe edilemedi, ultrason fizik muayeneyi doğruladı. Sol inguinal transvers insizyon yapıldı herni kesesi diseke edilirken sağ testis bulundu. Transseptal insizyon yapılarak testisler skrotuma indirildi.

Olgu 3: Sekiz yaşında erkek hasta sol palpe edilemeyen testis, sağ inguinal şişlik şikayeti ile başvurdu. Sol testis fizik muayenede palpe edilemedi, US ile gösterilemedi. Operasyonda, sağ inguinal transvers insizyon yapıldı sağ testisin herni kesesi diseke edilirken sol testis kese içinde bulundu. Ek olarak bu hastada duktus deferens füzyonu mevcuttu. Kese bağlandıktan sonra aynı yöntemle orşidopeksi yapıldı.

Olgu 4: İki yaşındaki hasta bilateral palpe edilemeyen testis nedeniyle başvurdu ek olarak özgeçmişinde sol inguinal herni operasyonu öyküsü mevcuttu. Fizik muayenede her iki testis palpabl değildi, skrotum gelişmemişti, sol inguinal transvers insizyon skarı mevcuttu. Ultrasonda her iki testis inguinal kanal girişinde batın içinde tespit edildi. Laparoskopide her iki testisin sağ internal ring hizasında karın içinde olduğu görüldü. Kord anomalisi ve mülleryan yapı yoktu, sağ inguinal insizyon yapıldı. Testislerin üzerinde kistik oluşumlar mevcut olduğu için biyopsi yapıldı. Fıtık kesesi bağlandıktan sonra transseptal orşidopeksi yapıldı. Biyopsi sonucu testis dokusu ile uyumlu geldi.

Tartışma:

Bu çalışmada, alışılmadık inmemiş testis şikayetleri ile başvuran dört “crossed ectopic testes” olgusu değerlendirildi. Hastalar palpe edilen, edilemeyen inmemiş testis, kasıkta sürekli olan şişlik şikayetleri ve fıtığa bağlı geçirilmiş inguinal cerrahi özgeçmişi ile başvurdu. Bütün hastalara aynı cerrahi teknik uygulandı, ektopik testisler kese içinde bulundu. Bir hastada duktus füzyonu mevcuttu. Hiçbir hastada mülleryan kanal anomalisi saptanmadı. Alışılmadık inmemiş testis bulguları ile başvuran hastalarda “crossed ectopic testes” göz ardı edilmemelidir.
