Retrospective Evaluation of Cases Pressure Ulcer in the Pediatric Intensive Care Unit

Introduction

The necrosis of both the cutaneous and the subcutaneous tissues of the   bony prominences of the body is called pressure (decubitus) ulceration. With the presence of repeated external compression and circulatory collapse, tissue ulceration occurs in the long term follow-up of the Intensive Care Unit (ICU) patients.

Materials and Methods

In this retrospective study, we analyzed children in the Pediatric Intensive Care Unit of Kocaeli Derince Education and Research Hospital from January 2012 - May 2013.

Results

We observed 346 pediatric patients; among which, 140 (40%) were female, 206 (60%) were male in the pediatric ICU. 345 (99%) of this 346 patients were admitted in the ICU for the first time, the rest 2 (0.6%)   were admitted for the second time. According to Braden risk scoring, we observed low ulcer development risk in 232 (67%) patients, moderate risk in 71 (20%) patients, high risk in 34 (10%) patients and very high risk in 9 (3%) patients, respectively.

 On admission to the intensive care unit, these patients had no decubitus ulcers. 2 (0.6%) patients developed pressure ulceration during follow-up .We detected this ulceration especially in the sacral region.  According to the degree of pressure sores these ulcerations were at stage 1 and 2, respectively. Pressure sores occurred on the 7th and 9th day of admission. Both patients had no surgical intervention and were treated with conservative methods.

Discussion

 Pressure ulcer development risk is low in pediatric age in spite of the existence of a prolonged hospitalization, rubbing, moisture, circulatory disorders, malnutrition, infection and anemia.  The usage of oxygen mask and nasal airway, as well as chair, seat cushion materials also cause such pressure sores.

For prevention of pressure ulcer development in the pediatric ICU, child's protection from infections, wound care of the child and family education and support is needed.

Çocuk Yoğun Bakım Ünitesinde Bası Yarası Olgularının Retrospektif Değerlendirilmesi

Giriş

Bası yarası; vücudun özellikle kemik çıkıntılarının bulunduğu bölgelerde uzun süreli ya da tekrarlayan basılara bağlı olarak cilt ve cilt altı dokularda kapillerlerin tamamen kapanması ve o bölgede dolaşımın durması sonucu ortaya çıkan nekroz ve ülserasyonlardır.

Gereç ve Yöntem

Kocaeli Derince Eğitim ve Araştırma Hastanesi Çocuk Yoğun Bakım Ünitesinde Ocak 2012- Mayıs 2013 tarihleri arasında takip edilen hastaların kayıtları retrospektif olarak incelendi.

 

Sonuçlar

Çalışmaya 140 (%40) kız, 206 (%60) erkek toplam 346 hasta alındı. Çocuk yoğun bakım ünitesine 345 (%99) hasta ilk kez, 2 (%0,6)  hasta ikinci kez kabul edildi. Branden risk değerlendirme ölçeğine göre bası yarası gelişme riski 232 (%67) hastada düşük, 71 (% 20) hastada orta, 34  (%10) hastada yüksek ve 9 (%3) hastada ise çok yüksek saptandı.

Çocuk yoğun bakım ünitesine kabul edilen hastalarda daha önceden bası yarası mevcut değildi. 2 (%0,6) hastada takip esnasında bası yarası gelişti. Bası yaraları sakral bölgede tespit edildi. Bu yaralar bası yarası derecelendirmesine göre 2 hastada da evre 1 olarak saptandı. Bası yaraları 7 ve 9. günlerde meydana geldi. Her iki hastaya da cerrahi müdahale yapılmadı ve konservatif yöntemlerle tedavi edildi.

Tartışma

            Çocuk yaş grubunda bası yarası gelişim riski düşük olmasına rağmen uzun süreli yatış, sürtünme, nem, dolaşım bozukluğu, malnütrisyon, enfeksiyon ve anemi durumlarında ve oksijen maskesi, nazal airway, sandalye, oturma yastığı gibi materyallere bağlı bası yarası gelişebilir.

Çocuk yoğun bakım ünitelerine alınan hastalarda bası yarası gelişimini önleyecek tedbirler alınmalı, bası yarası gelişen durumlarda çocuğun enfeksiyonlardan korunması, yara bakımının yapılması, çocuk ve ailesinin eğitimi ve desteklenmesi gereklidir.
