Is B-Flow USG superior to Color Doppler USG for evaluating blood flow patterns in ovarian torsion?

Objective:Ovarian torsion is common gynecological emergency that most often affects women of reproductiveage Its signs and symptoms are similar those of other abdominal conditions,which makes its differential diagnosis challenging.ColorDoppler(DopplerUSG) and B-flowultrasonography(B-flowUSG) are used for differential diagnosis of ovarian torsion,and in this study,we aimed to evaluate these two techniques and show that B-flowUSG is superior to colorDopplerUSG.
Materials and Methods:Sixteen rabbits of the same age and weight were separated into 2 subgroups of 8.In Group I,rightovary was fixated,and in Group II,rightovary was torsioned 720°counter clockwise and fixated.At 1hour(h) and 2h of ischemia, both ovaries were measured by two techniques,and an additional measurement was taken at 24h to determine any changes in the leftovary due to rightovary ischemia.Volume flow, peaksystolicvelocity(PSV),enddiastolicvelocity(EDV),resistiveindex(RI)and pulsatilityindex(PI) measurements were taken both with colorDopplerUSG(using 12MHz linear probe,General Electric medicalsystems logic7,Milwaukee,United States of America)and with B-flow USG.
Results:RightDopplerRI and PI values at 1h were significantly higher than right B-flow values(p<0.05).Difference between left B-flowRI and PI values and leftDoppler values at 1h was not significant(p>0.05),nor was the difference between rightB-flowRI value at 2h and rightDoppler RI value at 2h(p>0.05).RightB-flowPI value at 2h was statistically different from rightDoppler PI value at 2h(p<0.05),although the differences between leftB-flowRI and PI values at 1h and 2h and leftDopplerRI and PI values were not(p>0.05).However, there was a significant difference between RI values at 24h(p<0.05).
Conclusions:In this study,the decrease in blood reperfusion at 1h measured by B-flowUSG was apparently higher than that measured by DopplerUSG. However, by the end of 2h, the measurements were equal (see Fig.1and2). In conclusion,B-flowUSG reveals the decrease in the blood flow more distinctly in early phase. Moreover,B-flow DopplerUSG is more reliable and advantageous than color DopplerUSG because it provides more accurate evaluation of the unilateral and contralateral preoperative adnexal structures and allows for better assessment of blood flow in the ovarian torsion.

Amac: Over torsiyonu female popülasyonun jinekolojik acil cerrahisinde sıklıkla görülen hayati bir bozukluktur. Tanısı ve tedavisinin yönlendirilmesinde diğer akut batın hadiselerinden ayrımı oldukça zordur. Bu ayrıma giderken renkli doppler usg ve B-flow ultrasonografi kullanılmaktadır. Bu calismada over torsiyonunun tanısinda Blood flow ultrasonografi(B-flow USG)’nin Doppler Ultrasonografi (Doppler USG)’ye üstünlüğünü araştırmak üzere deneysel bir çalışma yapılmıştır
Gerec ve Yontemler: Aynı yaş grubu ve ağırlıkta 16 tavşan seçildi, 8er tavşandan oluşan iki gruba ayrıldı. Grup-I: Sağ over fikse edilip, Grup-II: Sağ over 720 ° saat yönünün tersine torsiyone edilip fikse edilerek 1 ve 2 saatlik iskemiler sonrası sol overle beraber eş zamanlı ölçümleri yapıldı, ancak sağ over iskemisinin sol overde yaratabileceği değişikliklerin daha iyi gözlenebilmesi açısından sol over 24. Saatte ek ölçüm yapıldı. General Electric medical systems logic 7 (Milwaukee, United States of America) ultrasonografi cihazı ile 12 MHz lineer probe kullanılarak renkli doppler ve B-flow usg ile volüm flow, peek systolic velocity (PSV), end diastolic velocity(EDV), resistiveindex(RI), pulsatilityindex(PI) bakıldı.
Bulgular: Sag Dopler 1.Saat RI ve PI değerleri, Sag Bflow 1. Saat ölçüm değerine göre anlamlı derecede daha yüksektir (p<0.05). Sol Bflow 1. Saat RI ve PI değerleri ile Sol Dopler 1.Saat arasında istatistiksel olarak anlamlı derecede farklılık görülmemektedir(p>0.05). Sag Bflow RI 2. Saat ile Sag Dopler RI 2.Saat arasında istatistiksel olarak anlamlı derecede farklılık görülmemektedir(p>0.05). Sag Bflow PI 2. Saat ile Sag Dopler PI 2.Saat arasında istatistiksel olarak anlamlı derecede farklılık görülmektedir(p<0.05). Sol Bflow 1. Ve 2. Saat RI ve PI ile Sol Dopler arasında istatistiksel olarak anlamlı derecede farklılık görülmemektedir(p>0.05). Ancak 24.Saat RI değerleri arasında istatistiksel olarak anlamlı derecede farklılık görülmektedir(p<0.05).
Sonuc: B flow doppler usg over torsiyonu vakalarında unilateral ve kontrlateral preoperatif adneksiyal yapılarının değerlendirilmesinde görüntü penceresinin genişliği ve damar içi kan akımlarının daha sağlıklı değerlendirilmesi açısından color doppler usge göre daha üstün özelliklere sahip ve daha güvenilir bir tekniktir.
