Evaluation of Bacterial Adhesion to Multifilament Sutures: in vitro study

Background/Aim: Several types of sutures and inorganic support materials are frequently used in surgical repairs. These foreign materials may cause foreign body reaction, bacterial colonization and biofilm formation. Although there are several studies regarding other support materials, there are very limited number of studies about sutures. Therefore, an in vitro study was conducted in order to investigate the bacterial adhesion to several types of multifilament sutures.

Materials/Methods: Totally 5 suture types were included to the study and allocated into groups as: polyglactin (Vicryl®) group (VG), furacin-coated polyglactin (Vicryl®) group (FVG), rapid absorbable polyglactin (Rapid-Vicryl®) group (RVG), polyethylene terephthalate (Etibond®) group (EG) and natural silk (Silk®) group (SG). All sutures were cut in 1 cm length and embedded into tryptic soy-broth agar. 106 cfu/ml Escherichia coli ATCC 25922 was added to all of them. 10 samples were studied in each group. After 24 hr incubation, sutures were washed with sterile phosphate solutions twice and sonificated. Ten times diluted samples were taken from the sonificated fluids, cultivated in tryptic soy-broth agar and 24 hr later colonies were counted. The colonies were counted at 1st, 3rd and 7th days of incubation. Results were evaluated statistically.

Results: The mean of colonies were 7.4x103 (3.4x103- 1.3x104) in VG, 2.7x104 (2.5x104- 3.0x104) in FVG, 3.7x104 (1.4x104- 5.2x104) in RVG, 1.7x104 (7.9x103- 3.2x104) in EG and 8.2x104 (7.0x104- 9.5x104) in SG. Bacterial adhesion was significantly lower in VG and significantly higher in SG compared to FVG, RVG and EG (p<0.05).  

Conclusion: Of all multifilament sutures, bacterial adhesion is significantly lower in polyglactin and significantly higher in silk sutures. It does not cause a significant difference in anti-bacterial efficiency if the suture is coated with furacin, rapidly absorbed or never absorbed.  

Örgülü Sütür Materyallerinde Bakteri Adezyonunun in vitro Değerlendirilmesi

Giriş/Amaç: Cerrahi onarımlarda çeşitli sütür materyalleri ve yardımcı inorganik malzemeler kullanılmaktadır. Bu malzemelerin yabancı cisim reaksiyonu, malzeme üzerinde bakteri kolonizasyonu ve biofilm oluşumu gibi istenmeyen etkileri görülebilmektedir. Diğer destek malzemeleri ile ilgili çok sayıda araştırma olmasına rağmen sütürlerle ilgili çalışmalar sınırlı sayıdadır. Buradan yola çıkarak cerrahi onarımlarda kullanılan örgülü farklı sütür materyallerinin bakteri adezyonuna olan yatkınlıklarını belirlemek amacıyla in vitro bir çalışma planlanmıştır.

Gereç/Yöntem: Toplam 5 adet multifilament sütür kullanıldı. Kullanılan sütürler şu şekilde gruplandı: polyglactin (Vicryl®) grubu (VG), furacin emdirilmiş polyglactin (Vicryl®) grubu (FVG), hızlı emilen polyglactin (Rapid-Vicryl®) grubu (RVG), polyethylene terephthalate (Etibond®) grubu (EG) ve ipek (Silk®) grubu (SG). Sütürler 1 cm. uzunluğunda kesilip triptik soy-broth kültür pleytlerine yerleştirildi ve 106 cfu/ml Escherichia coli ATCC 25922 suşu eklendi. Her gruptan 10 örnek değerlendirildi. 24 saatlik sürenin sonunda sütürler steril fosfat tamponlu su ile iki kez yıkandı, sonifikiye edildi. Sonifikasyon sıvısından 10 katlık seri dilüsyonlar hazırlanarak triptik soy agara ekildi. 37°C’de 24 saatlik inkubasyon sonunda koloni sayımları yapıldı. Koloni sayıları 1, 3 ve 7.günlerde ölçülüp gruplar arasında karşılaştırıldı.

Bulgular: Sütür gruplarında 24 saatlik inkubasyon sonunda ortalama koloni sayıları; VG’da 7.4x103 (3.4x103- 1.3x104), FVG’da 2.7x104 (2.5x104- 3.0x104), RVG’da 3.7x104 (1.4x104- 5.2x104), EG’da 1.7x104 (7.9x103- 3.2x104) ve SG’da 8.2x104 (7.0x104- 9.5x104) olarak saptandı. Gruplar karşılaştırıldığında bakteri adezyonunun VG’da diğer gruplara göre anlamlı derecede az, SG’da ise anlamlı derecede fazla olduğu görüldü (p<0.05).  

Sonuç: Bakteri adezyonu örgülü özellikteki sütür materyallerinden polyglactin’e en az, ipek sütüre ise en fazla olmaktadır. Sütürün furacin kaplanmış olması, hızlı emilmesi ya da emilmemesi anti-bakteriyel etkinliğe ek bir etki sağlamamaktadır.
