POSTTRAUMATIC HEMATURIA EVALUATION: ACUTE INCIDENTAL GLOMERULONEPHRITIS

Introduction:

The cause for eighty percent (%80) of patients admitted to pediatric emergency services with hematuria is trauma. We aimed to discuss a case which is admitting the emergency service and diagnosed as post-infectious glomerulonephritis. 

Presentation:

A 7-year old boy has admitted to emergency service as result of hematuria occurred following the trauma to the right costovertebral and suprapubic locations.

Any abnormality was not detected in terms of the systems after the example of a complete physical. Blood biochemical values were normal while abundant erythrocytes were observed in the urine. In urinary tract ultrasonography diffusely increased echogenicity of the right kidney was seen. If bladder wall thichness was normal while a hematoma changing localization and forming air-fluid level was seen.

Following retrograde cystourethrogram imaging, extravasation was not observed in the bladder and urethra of the patient.

Patient was hospitalized, and then urinary bladder irrigation was done in order to drain the hematoma with saline solution. In control biochemical analysis performed depending on continuation of gross hematuria, urea and creatinine levels are detected as 64 mg/dl and 1.14 mg/dl, while 12 hours later the values have reached 78 mg/dl and 1.30 mg/dl respectively. Following the example of a complete history, it was detected that patient has suffered upper respiratory infection prior four weeks. Therefore, patient was consulted with the department of pediatrics. 

Post-infectious glomerulonephritis is diagnosed with further examinations and kidney biopsy.

Discussion:

It should be kept in mind that a hematuria occurred after trauma may not be related with urinary tract trauma, and such cases should be evaluated in terms of internal pathologies also. 

TRAVMA SONRASI HEMATÜRİ DEĞERLENDİRMESİ: RASTLANTISAL AKUT GLOMERÜLONEFRİT

Giriş:

Hematüri nedeniyle çocuk acil servislerine başvuran olguların %80’inde neden, travmaya bağlıdır. Burada  travma sonrası hematüri şikayeti ile acil servise başvuran ve takip sürecinde postenfeksiyoz glomerülonefrit tanısı konulan bir olgu tartışılmaktadır.

Olgu Sunumu:              

Yedi yaşında erkek hasta sağ kostovertebral ve suprapubik bölgesine aldığı darbeler sonrasında gelişen hematüri nedeniyle acil servise başvurdu.

Olgunun yapılan fizik muayenesinde tüm sistem bulguları doğal olarak değerlendirildi. Kan biyokimyasına ait değerleri normal,tam idrar tahllinde ise bol eritrosit izlendi. Üriner sistem ultrasonografisinde sağ böbrek parankim ekosu diffüz olarak artmış , mesane duvar kalınlığı normal görünümde olup içinde hareket ile yer değiştiren, seviyelenme oluşturan hematom mevcuttu.

Retrograd sistoüretrogram uygulanan hastada üretra ve mesaneden herhangi bir ekstravazasyon gözlenmedi.

Olgu kliniğe yatırıldı. Mesane kateterizasyonu sağlanarak hematomun boşalması için serum fizyolojik ile mesane irrigasyonuna başlandı.

Gross hematürisinin devamı üzerine yapılan kontrol biyokimyasal incelemesinde Üre 64 mg/dl,Kreatinin 1,14mg/dl 12 saat sonraki kontrollerinde ise Üre 78 mg/dl,Kreatinin 1,30mg/dl olduğu görüldü. Hasta tekrar sorgulandığında yaklaşık 4 hafta önce geçirilmiş üst solunum yolu enfeksiyonu tariflenmesi üzerine olgu çocuk nefroloji kiniğine konsülte edildi.

 Yapılan ileri tetkikler ve böbrek biyopsisi sonucu postenfeksiyoz glomerülonefrit tanısı konuldu.

Tartışma:

Travma öyküsü olan acil servise başvuran olgulardaki hematüri, her zaman üriner sistem travmasına bağlı olmayacağı akılda tutulmalı ve olgular dahili patolojiler açısından da daha ayrıntılı sorgulanmalıdır.
