Ovarian pathologies from newborn to adolescent period
Background and Aim: Ovarian cysts can be seen in every age with several clinical manifestations and different pathological types. In this study, we aimed to determine the clinical and histopathology features in cases with ovarian lesion operated at our department. Material and Method: All records of cases with ovarian lesions, operated at our department between January 2004 and June 2014, were reviewed retrospectively. Demographic features, complaints, radiologic results, tumor markers, operative method, histopathology results and hospitalization times were analyzed. Results: During 11-year 6-month period, 72 girls were operated. 12 cases (18%) were smaller than 45 days (1–37 day); and mean age for other 60 cases (82%) were 145.71±51.41 months. Ovarian lesions were located right in 44 (61%), left in 24(34%), and bilateral in 4(5%). There were 76 ovarian units affected. Preoperative tumor markers were negative in 70 cases. The value of the blood ca125 was found elevated minimally in two cases, and in one of them histopathology diagnosis was benign mucinous cystadenoma. Cases were operated with open technique in 41 (57%), and with laparoscopicaly in 26 (36%), and with laparoscopy assisted 5 (7%). Cystectomy, oophorectomy, or salpingo-oophorectomy was performed. Histopathology revealed unidentified tissue with hemorrhagic necrosis, mature cystic teratoma, struma ovarii, disgerminoma, corpus luteum cyst, and simple ovarian cyst. Pathological specimen was not obtained in 9 cases. Mean hospitalization time was 2,4±1,28 days. Conclusion: Ovarian pathology in childhood most often are seen two peaks in newborn and puberty. Although spontaneous regression is possible in intrauterine diagnosed ovarian cyst, some of them may result in torsion and loss of over. During the pubertal period spectrum of ovarian lesions can be range from simple cysts to mature cystic teratoma or disgeminoma. Negative tumor markers are not excluding neoplastic lesions; fortunately most of the neoplastic cysts are benign in childhood period.
