Inflammatory myofibroblastic tumor of the bladder: Case report

Objective: We present a case of female with an Inflammatory Myofibroblastic Tumor (IMT) of the bladder treated with partial cystectomy with uneventhful post-surgical course.

Case Report: A 14 years old female referred with a history of gross hematuria and a mass measured 11x12x13 cm. at the dome of the bladder, with the inconclusive pathological examination of specimen taken during the exploratory laparotomy. Re-examination of the specimen revealed the IMT or rhabdomyosarcoma of the bladder. The patient was referred to our institution for further management of a suspected rhabdomyosarcoma. After a diagnostic work up which included ultrasound scan and magnetic resonance, the patient underwent cystoscopy. The vegetative mass appeared originating from the anterior-superior part of the bladder wall and involve the entire nearly bladder cavity. Exploratory laparotomy with bladder-preserving surgical approach was performed. Pathologic examination revealed myxoid cells in patches, spindled, myofibroblastic and focal inflammatory cells with positive surgical margins. The diagnosis of IMT was confirmed with immunohistochemical stains. Postoperatively, the patient received anticholinergic regimen for reduced bladder capacity and nonsteroidal anti-inflammatory regimen for a total of 6 months. At 6 months of follow-up, the patient was doing well with no pain, hematuria or voiding complaints. No recurrence or progress of the tumor in the bladder wall detected radiologically

Result: Inflammatory Myofibroblastic Tumor of the bladder can be confused with rhabdomyosarcoma during radiological evaluation. Resection of the lesion with partial cystectomy and post-operative nonsteriod anti-inflammatory regimen may result a favourable outcome. 

Mesanenin İnflamatuvar Myelofibroblastik Tümörü; Olgu Sunumu

Amaç: Bu sunumda mesanenin inflamatuvar myelofibroblastik tümörü (İMT) nedeniyle parsiyel sistektomi yapılan 14 yaşında kız hastayı sunmayı amaçladık.

Olgu Sunumu: Dış merkezde makroskopik hematüri öyküsü nedeniyle araştırılan 14 yaşındaki kız hastada mesanede 11x12x13 cm boyutlarında kitle saptanmıştı. Laparatomi ile biyopsi yapılarak rabdomyosarkom şüphesiyle merkezimize refere edildi. Merkezimizde spesmenlerin tekrar değerlendirilmesi sonucunda mesane rabdomyosarkomu ya da İMT ile uyumlu olabileceği düşünüldü. Tanısal amaçlı ultrason, manyetik rezonans görüntüleme ve sistoskopi yapıldı. Mesane anterior superior duvarından kaynaklanan hemen hemen tüm mesane lümenini kaplayan vejetatif kitle saptandı.  Laparatomide trigonu içerecek şekilde mesane koruyucu cerrahi uygulandı. Patolojik incelemede cerrahi sınırlar pozitifti. Yer yer miksoid, iğsi, myelofibroblastik ve fokal inflamatuvar hücreler izlendi. İnflamatuvar myelofibroblastik tümör tanısı immunohistokimyasal boyalarla doğrulandı. Postoperatif dönemde mesane kapasitesini artırmak için antikolinerjik  ve inflamatuvar hücrelere yönelik nonsteroid anti-inflamatuvar rejim 6 ay uygulandı. Altı aylık takipte ağrı, hematuri ya da işeme problemi olmadı. Radyolojik tetkiklerde mesane duvarında rekürrens ve progresyon izlenmedi.

Sonuç: Mesane kaynaklı inflamatuvar myelofibroblastik tümör rabdomyosarkomla karışabilir. İMT’de parsiyel sistektomi ve nonsteroid anti-inflamatuvar rejimle başarılı sonuçlar elde edilebilir.
