A large Mullerian Duct Cyst located in retrovesical space mimicking hydatid cyst

Objective: We herein present a case of male with a large retrovesical mullerian ductal cystic mass that radiological evaluation gives an impression of hydatid cyst, operated via laparoscopically successfully.

Case Report: A 17 years old male presented with complaints of intermittent abdominal pain. An ultrasound exam of the abdomen showed a large muiti-septated cystic lesion posterior to the bladder. A computed tomography scan was done, which showed a large cystic lesion behind the bladder. Both radiological evaluations gave an impression of hydatid cyst with a differential diagnosis of seminal vesical cyst. Laporoscopical examination revealed a large cystic mass located behind the bladder. Csytic mass was removed via laparocopically with aspiration of the cyst and removing of the cyst wall. A drain was left in the space. Pathological examination of the cyst wall reported as mullerian duct cyst. The postoperative period was uneventful.

Result: Cyst hydatid is a well-known abdominal cystic lesion in children in endemic regions, easily diagnosed with radiological examinations. Due to the rare nature of mullerian duct cyst in males, a high index of suspicion is necessary for diagnosis. Laporoscopical exploration remains the mainstay of diagnosis in case of confusion.

Kist hidatiği taklit eden retrovezikal yerleşimli büyük Mülleriyan Kanal Kisti

Amaç: Laporoskopik olarak başarılı şekilde opere edilen, kist hidatiği taklit eden, retrovezikal yerleşimli büyük mülleriyan kanal kistli olgu sunulmaktadır.

Olgu sunumu: On yedi yaşında erkek hasta aralıklı karın ağrısı yakınmasıyla başvurdu. Ultrason incelemesinde mesane posteriorunda büyük multiseptalı kistik lezyon saptandı. Pelvik bölgenin bilgisayarlı tomografi incelemesinde mesane posteriorunda kistik lezyon saptandı. Radyolojik ön tanı kist hidatik ile uyumluydu. Ayırıcı tanıda   seminal vezikül kisti düşünüldü. Laparaskopik incelemede mesane arkasında büyük kistik kitle görüldü. Laparaskopik olarak kist aspire edildi ve kist duvarı çıkarıldı. Operasyon lojuna dren yerleştirildi. Kistin patolojik değerlendirmesi mülleriyan kanal kistiyle uyumluydu. Postoperatif dönemde sorun olmadı.

Sonuç: Kist hidatik endemik bölgelerde sık görülen bir kistik kitledir. Nadir görülen mulleriyan kanal kisti karında farklı lokalizasyonlarda izlenebilir. Mülleriyan kanal kisti erkeklerde nadirdir. Tanı karışıklığı olan bu tür olgularda laparaskopik değerlendirme ilk basamak olabilir.
