Inflammatory Myofibroblastic Tumor in an Unusual Location

Inflammatory myofibroblastic tumor (IMT) is a benign to low grade malignant, fibroblastic tumor that is commonly seen in adolescents.  Even it is mostly localized in thorax, abdomen and retroperitoneum IMT can be seen at other parts of the body and may lead to different clinical pictures.

A 10-year-old boy was admitted with a history of rapidly growing, painless mass located in the right buttock which was noticed by his mother one month ago. There were no previous history of trauma or sign of inflammation over the mass. Physical examination revealed a protruding nontender immobile solid mass located in the right gluteal muscles and neighbouring anus, measuring 4 cm in diameter. A magnetic resonance imaging study showed a solid and well encapsulated hypodense mass, 5x3x4cm in diameter, which was noninvasive to surrounding structures. The mass was surgically explored with presumptive diagnoses of germ cell tumor or a tumor of muscular origin. Operative findings showed a white-yellow colored, firm in consistency and well encapsulated mass in a diameter of 3,5x3,5x2,5 cm with clear surgical margins. The mass was resected totally. The histopathologic diagnosis was compatible with IMT. Patient had an uneventful postoperative period and was discharged on the postoperative day 4. A follow-up examination revealed no sign of recurrence 6 months after the operation.

IMT is a rare fibroblastic tumor presenting with various clinical pictures and causing unpredictable outcomes. Gluteal region is an extremely rare site for localization of IMT.  Although rare, IMT should be kept in mind for differential diagnosis of a solid tumor of musculoskeletal system. Since it yields cure and better prognosis, complete excision should be the goal.

Alışılmadık Yerleşim Gösteren İnflamatuar Myofibroblastik Tümör

İnflamatuar myofibroblastik tümör (IMT), benign veya düşük derecede malign bir tümör olup adolesanlarda daha sık görülür. Çoğunlukla göğüste, karında ve retroperitonda yerleşmesine rağmen vücudun diğer kısımlarında da görülebilir ve değişik klinik bulgulara neden olabilir.

On yaşındaki erkek hasta 1 ay önce annesi tarafından farkedilen sağ kalçadaki ağrısız, hızla büyüyen kitle nedeni ile başvurdu. Travma öyküsü ve kitle üzerinde inflamasyon bulgusu yoktu. Fizik muayenede, sağ gluteal bölgede anüs komşuluğunda yerleşmiş, 4 cm. çaplı, hassas olmayan immobil kitle tespit edildi. Magnetik rezonans incelemede solid, kapsüllü ve çevre dokulara invaze olmamış, 5x3x4cm. çapındaki hipodens kitle görüntülendi. Kas kaynaklı veya germ hücreli tümör öntanısı ile operasyona alınan kitle beyaz-sarı renkte, sert, kapsüllü ve 3,5x3,5x2,5 cm. çapta idi. Kitle temiz cerrahi sınırlarla tamamen çıkartıldı. Histopatolojik olarak IMT ile uyumlu idi. Hasta postoperatif 4. günde sorunsuz olarak taburcu edildi. Hastanın 6 aylık takibinde nüks bulguları ile karşılaşılmadı.

IMT, çeşitli klinik bulgularla kendini gösterebilen ve öngörülemeyen sonuçlara yol açabilen nadir bir fibroblastik tümördür. Gluteal bölge IMT için nadir bir yerleşim bölgesidir. IMT nadir olmasına rağmen, kas iskelet sisteminin solid tümörlerinin ayırıcı tanısında akılda tutulmalıdır. İyileşme ve iyi bir prognoz sunan tam eksizyon tedavideki amaç olmalıdır.
