Chylothorax developing after esophageal atresia repair: Case report

Introduction
Esophageal atresia (EA) is uncommon in the development of chylothorax after repair. The experience related to chylothorax that developed in a newborn due to surgical trauma is shared.


A Case Report
Before the operation the echocardiography of the female patient that been born by c-section at 34 weeks and 2120 grams with the diagnosis of proximal-distal tracheo-esophageal fistula, esophageal atresia (PEA-dTEF) showed tetralogy of Fallot and left aortic arch. During the surgery right posterolateral thoracotomy incision was performed. The right aortic arch was present and proximal esophagus was under the arch. Esophagus mobilization performed hardly through under the aortic arch. TEF ligation was performed with primary repair of esophageal atresia. Yield of postoperative chest tube was about 30 cc/kg /day, and in chylous quality. Biochemical analysis of the fluid compatible with Chylothorax . From the 11th day, enteral product that contains low-fat is initiated. The 18th day, intravenous octreotide initiated. Chylous yield was in the range of 11-40 cc/kg/day Thoracotomy was performed 36 days after the first surgery, decortication and fibrin glue was applied to the surgical area. Postoperative period passed without any problem. Now, after seven months from the surgery and yet there is no problem.


Result
Surgical trauma is a rare cause of chylothorax in neonatal. In the patient presented in here it is possible that thoracic duct is injured because of the hard dissection of the proximal esophagus that settled under the arch. In the cases with EA The presence of the aortic arch that turns from right, surgical procedure from the left side can be preferred. However, as in this case, preoperative echocardiography can provide misleading results. The response of iatrogenic chylothorax as in this case is poor to the conservative treatment. Because of the small number of cases, there is no conclusive recommendation for how long the conservative treatment should be continued. However, with a neonatal that had major surgery, and have additional abnormalities with insufficient enteral nutrition, both early secondary surgery and unnecessary time loss related to conservative treatment should be avoided

Özofagus atrezisi onarımı sonrası gelişen şilotoraks: Olgu sunumu

Giriş

Özofagus atrezisi (ÖA) onarımı sonrası şilotoraks gelişimi nadir görülür. Bir yenidoğanda cerrahi travmaya bağlı gelişen şilotoraks ile ilintili deneyim paylaşılacaktır.

Olgu Sunumu

C/S ile 34 haftalık ve 2120 gram doğan proksimal ÖA-distal trakeo-özofageal fistül (pÖA-dTÖF) tanılı kız hastanın ameliyat öncesi ekokardiyografisinde Fallot Tetralojisi ve sol arkus aorta saptandı. Ameliyatta sağ posterolateral torakotomi insizyonu yapıldı. Sağ arkus aorta mevcuttu ve özofagus proksimali arkusun altında idi. Arkus aorta altından özofagus mobilizasyonu güçlükle yapıldı. Primer ÖA onarımı ile TÖF ligasyonu gerçekleştirildi. Postoperatif toraks tüpü getirisi yaklaşık 30 cc/kg/gündü ve şilöz vasıftaydı. Sıvının biyokimyasal incelemesi şilotoraksla uyumluydu. 11. günden itibaren düşük yağ içerikli enteral ürün başlandı. 18. günde intravenöz oktreotid başlandı. Şilöz getirisi 11-40 cc/kg/gün aralığındaydı. İlk ameliyatından 36 gün sonra tekrar torakotomi yapılarak dekortikasyon ve loja fibrin yapıştırıcı uygulandı. Ameliyat sonrası dönem sorunsuz seyretti. Halen ameliyat sonrası 7. ayında ve sorunsuzdur.

Sonuç

Yenidoğanlarda cerrahi travma az görülen bir şilotoraks nedenidir. Burada sunulan hastada arkusun altında yerleşmiş proksimal özofagusun güç diseksiyonu nedeniyle torasik duktusun yaralanmış olması muhtemeldir. ÖA’lı olgularda sağdan dönen arkus aorta varlığı cerrahinin soldan yapılması için bir tercih nedeni olabilir. Ancak bu olguda olduğu gibi, ameliyat öncesi yapılan ekokardiyografi yanıltıcı sonuçlar verebilmektedir. İyatrojenik şilotoraksın, bu olguda olduğu gibi konservatif tedaviye yanıtı azdır. Olgu sayısının azlığı nedeniyle, konservatif tedavinin ne kadar sürdürülmesi gerektiği konusunda kesin öneriler yoktur. Bununla birlikte, majör cerrahi girişim geçirmiş, ek anomalileri olan ve yeterli enteral beslenmesi sağlanamayan bir yenidoğanda hem gereksiz erken sekonder cerrahiden kaçınılmalı hem de konservatif tedaviyle gereksiz zaman kaybedilmemelidir.
