Abdominal tuberculosis; report of 5 cases

Introduction

Tuberculosis is a systemic infection which Mycobacterium Tuberculosis causes it. It is paid attention that extra pulmonary tuberculosis cases were increased beside pulmonary tuberculosis cases in recent years. Diagnosing of  abdominal tuberculosis is very diffucult which usually can be diagnosed after surgery. In this report, 5 cases were presented that one of them was operated for intraabdominal mass for diagnose and 4 of them who had abdominal tuberculosis lymphadenitis after surgery were operated for acute abdomen.  

 

CASES:

Five cases'  who had abdominal tuberculosis mean age was 13(8-17) and 2 of the 5 cases were male and 3 of them were female. Of the 5 cases,  four cases were consulted for abdominal pain in emergency unit and one was consulted for abdominal mass in infectious diseases clinic. All cases had loss of weight. Four cases had acute abdomen one of them was operated after physical examination, one of them was operated after abdominal ultrasonography and abdominal tomography, and two of them were operated after ultrasonography. We diagnosed tuberculosis in 4 cases because of caseification necrosis in the surgical operation that we did not expect it before surgery.Tuberculosis was diagnosed by histopathological examination in all cases but  culture was positive for mycobacterium tuberculosis only in one case. All cases received anti-tuberculosis therapy as a result of clinical and surgical diagnose before obtaining definitive histopathological diagnosis. One case was followed and treated for high volume ascites for long time in hospital in early postoperative period  others were discharged uneventfully.

 

Conclusion:

Abdominal tuberculosis is very hard diagnosing  pathology which mimicks various pathologies by non-spesicific symptoms and abdominal pain attacks.

Definitive diagnosis is only possible with histopathologic examination. Although diagnosis can be made by sample cultures which were taken from body secretions but results of these samples could be obtained in late period.  In this pathology early diagnose and treatment is very important for this reason abdominal tuberculosis must be kept in mind  in suspicious patients  who were operated for acute abdomen, and tissue samples must be taken for definitive diagnosis especcially from lypmh nodes.

Abdominal tüberküloz; 5 olgu sunumu

Giriş:

Tüberküloz, Mycobacterium Tuberculosis basilinin neden olduğu sistemik bir enfeksiyondur.  Son yıllarda akciğer tüberkülozu yanında akciğer dışı tüberküloz olgularında da artış olduğu dikkati çekmektedir. Abdominal tüberküloz tanısı oldukça güç olup, genellikle cerrahi sonrası konulabilmektedir.   Burada, karında kitle nedeniyle tanısal laparotomi yapılan bir hasta ve akut karın nedeniyle ameliyat edilerek  abdominal tüberküloz lenfadenit tanısı alan 4 hasta sunulmuştur.

Olgular:

Abdominal tüberküloz tanısı alan 5 olgunun 2’si erkek, 3’ü kız olup ortalama yaş 13 (8-17 yaş) idi. 4 olgu karın ağrısı nedeniyle acil ünitesinden, 1 olgu karında kitle nedeniyle çocuk enfeksiyon hastalıkları servisinden konsülte edildi. Tüm olgularda kilo kaybı vardı.  Akut karın tablosu olan hastaların biri fizik muayene sonrası, biri abdominal USG ve abdominal BT, ikisi de USG yapıldıktan sonra ameliyat edildi. Ameliyat öncesi abdominal tüberküloz tanısı düşünülmeyen 4 olguda, ameliyat sırasında görülen kazeifikasyon nekrozu öncelikle tüberkülozu düşündürdü. Tüm olgulara histopatolojik olarak tanı konuldu, sadece bir tanesinin kültüründe mucobacterium tuberculozis üretilebildi.  Tüm olgulara klinik öntanı nedeniyle histopatolojik kesin tanı öncesi  anti-tüberküloz tedavi başlandı. Bir olgu post-operatif erken dönemde karında belirgin asit olması nedeniyle uzun süre hastanede izlendi, diğer olgular sorunsuz olarak taburcu edildi.

Sonuç:

Abdominal tüberküloz; karın ağrısı atakları ve non-spesifik semptomlarla çeşitli patolojileri taklit edebilen tanısı oldukça güç bir hastalıktır. Kesin tanı ancak histopatolojik inceleme ile mümkündür.  Vücut sıvılarından alınan tbc kültürleri ile tanı konabilmekteyse de sonuçlar geç dönemde elde edilmektedir. Erken tanı ve tedavi çok önemli olduğu için, akut batın tanısı ile opere edilen şüpheli olgularda abdominal tüberküloz  akılda tutulmalı ve özellikle lenf nodlarından olmak üzere tanısal amaçlı doku örnekleri alınmalıdır.
