choledochal cyst in children

~~Aim:  We aimed to present preoperative and postoperative assessments of patients who were operated for choledochal cyst.
Materials and Methods: The records of patients who were operated between January 2005 and June 2013 for choledochal cyts in our hospital were analyzed retrospectively.
Result: Of the patients, 6 were male and 10 were female. Ages were between 3 months and 17 years (mean 6,2). All patients present with abdominal pain. 4 patients presented with recurrent episodes of jaundice, right upper quadrant pain and a palpable mass in the right upper quadrant. Blood chemistry and complete blood count were performed. Ultrasound was performed in all patients. Biliary stone/sludge and intrahepatic bile duct dilatation were detected additionally, in 7 and 13 patients respectively. MRCP was performed as further imaging studies in all patients. In all, 11 patients (69%) were suggested to have type I cysts, 1 (%6) type II, 1 (%6) type III, and 3 (19%) were suggested to have type IVa according to Todanı’s classification preoperatively. The average cyst diameter was 4.7 cm. We performed computed tomography and upper gastrointestinal contrast study in one patient to rule out duodenal duplication cyst. Preoperatively 6 of 16 patients had percutaneous transhepatic biliary catheterization for drainage. In all but one patient, total cyst excision and Roux-en-Y hepaticoenterostomy were chosen. Type III cyst was treated by duodenotomy and cyst excision. In the early postoperative period, ileus and bile leakage were detected in 1 and 1 patients respectively.  The average time of hospital stay was 7.6 days in uncomplicated with patients. The patients were followed-up 6 months to 7 years (mean 3.43 years) uneventfully.  In the patients who were suggested to have Type IVa choledochal cysts intrahepatic dilatation was completely resolved after surgery which proved them to be Type I.
Conclusions: Cyst excision and hepaticoenterostomy is the preferred treatment for the choledochal cyst. We consider that dilatation of the intrahepatic bile duct seen preoperatively is due to stasis of bile which results from the obstruction of the distal common bile duct.
 

Çocuklarda Koledok kistleri

Amaç: Çalışmamızda koledok kisti nedeniyle ameliyat edilen hastaların ameliyat öncesi ve sonrası değerlendirmelerini sunmayı amaçladık.
Gereç ve Yöntem: Ocak 2005 ile Haziran 2013 tarihleri arasında hastanemizde koledok kisti tanısı ile ameliyat edilen hastaların kayıtları geriye dönük olarak incelendi.
Bulgular:  Yaşları 3 ay ile 17 yıl arasında (ortalama 6,2 yıl) değişen, 6’ sı erkek, 16 hasta vardı. Tüm hastalar karın ağrısı ile başvurdu. 4 hastada sarılık atakları, sağ üst kadran ağrısı ve sağ üst kadranda ele gelen kitle birlikteliği vardı. Bütün hastalarda kan biyokimyası ve tam kan sayımı yapıldı. Tüm hastalara ultrasonografi yapıldı. 7 hastada safra yollarında taş veya çamur, 13’ünde de intrahepatik safra yollarında dilatasyon saptandı. Ek görüntüleme yöntemi olarak mağnetik rezonans görüntüleme yapıldı. Todani sınıflamasına göre ameliyat öncesinde 16 hastanın 11’inde Tip I (%69), 1’inde Tip II (%6), 1’inde Tip III (%6) 3’ünde Tip IVa (% 19) ile uyumlu koledok kisti düşünüldü. Kist büyüklüğü ortalama 4,7 cm idi. Tip III koledok kisti ile duodenum duplikasyon kisti ayrımı için 1 hastamızda CT ve ÖMD gerekti. 6 hastamıza ameliyat öncesi perkütan transhepatik kateter takılarak biliyer sistem drenajı uygulandı. Tip III olan bir hasta dışında tüm hastalarda cerrahi yöntem olarak total kist eksizyonu+Roux-en-Y hepatikoenterostomi yapıldı. Tip III koledok kisti duodenotomi ile eksize edildi. 1 hastamızda erken dönemde ileus, bir hastada safra kaçağı gelişti. Komplikasyonsuz ameliyatlarda ortalama yatış süresi 7,6 gündü. Hastalarımızın 6 ay-7 yıl arasında (ortalama 3,43 yıl ) takip süreleri boyunca herhangi bir sorunları olmadı. Preoperatif olarak Tip IVa düşünülen hastaların ameliyat sonrası ultrasonografilerinde intrahepatik safra yollarında dilatasyon saptanmadı.      
Sonuç: Tedavide kist eksizyonu ile birlikte hepatikoenterostomi tercih edilen cerrahi yöntemdir. Hastalarımızda ameliyat öncesi saptanan intrahepatik safra yolları dilatasyonunun koledok alt ucunda obstrüksüyona bağlı safra stazı nedeniyle gelişmiş olduğu düşünüldü.
