ONEN CATHETER PLACEMENT IN CHILDREN UNDERGO PERCUTANEOUS NEPHROLITHOTOMY: VIDEO PRESENTATION

A 5 Fr Onen catheter has been placed in a 7-years-old child who underwent percutaneous nephrolithotomy (PCNL) for a large renal stone. At the end of surgery, the soft tip of a guidewire was inserted into the bladder through the percutaneous access sheath of the kidney. Then, the J-type distal tip of a 5 Fr Onen catheter was inserted into the bladder over this guidewire. The Malecot part of the catheter was placed into the renal pelvis. Thereafter, the renal access sheeth was removed, and the outside part of the catheter was fixed to the skin using a 3/0 silk suture and placed under the dressing of incision. Similar to JJ-stent, the Onen catheter promised to drain the urine from the kidney to bladder safely and prevented perirenal urine extravasation. In addition, it decreases the need of long lasting standard nephrostomy tube drainage due to upper urinary tract obstruction and pyelonephritis that occur because of possible residual stones.

Onen catheter was removed with no need of a second anesthesia in outpatient clinic at postoperative 1 week after proving no hydronephrosis and residual stone on ultrasound.

In conclusion, Onen catheter is safe, easy to perform and successful in children undergo PCNL. Not only Onen catheter prevents the need of a second anesthesia that require when using JJ-stent but also it decrease morbidity rate by promising to carry the urine from the kidney to bladder safely. 

PERKÜTAN NEFROLİTOTOMİDE ÖNEN KATETERİ YERLEŞTİRME: VİDEO SUNUMU

Yaklaşık 7 yaşında bir hastada böbrek taşı nedeniyle yapılan perkütan nefrolitotomi (PNL) operasyonu esnasında 5 Fr Önen kateteri yerleştirildi. Önce böbrekteki giriş kılıfı içinden böbreğe ve oradan da mesaneye klavuz telin yumuşak ucu gönderildi. Daha sonra, bu klavuz tel üzerinden böbrek, üreter ve mesane içine olmak üzere üriner sisteme 5 Fr Önen kateteri geçirildi. Önen kateterinin malekot bölümü renal pelviste ve J-şeklindeki distal ucu mesanede olacak şekilde üriner sisteme yerleştirildi. Daha sonra, deri giriş yerinden kateter cilde tespit edilip dış ucu kapatılarak pansuman içine alındı. Böylece, JJ-stente benzer şekilde idrarın perirenal alana dağılmadan ve güvenle mesaneye ulaşması sağlanırken, standart nefrostomi kateteri takılan olgularda olası rezidü taşların üst üriner sistemi tıkayarak pyelonefrit ve idrar ekstravazasyonu riski ve buna bağlı nefrostominin uzun süre bekletilme ihtiyacı azaltılmış oldu.

PNL’den 1 hafta sonraki USG’de üriner sistemde hidronefroz, rezidü taş vb patoloji olmadığı görüldükten sonra poliklinik şartlarında ve ikinci anestezi ihtiyacı duyulmadan Önen kateteri çekildi.

Sonuç olarak, PNL uygulanan olgularda Önen kateteri güvenle, kolaylıkla ve yüksek başarı ile uygulanabilir. Özellikle ikinci anestezi ihtiyacını ortadan kaldırması ve böbrekteki idrarı güvenle mesaneye aktararak morbidite riskini azaltması mevcut diğer kateterlerden üstün kılan en önemli avantajlarıdır.
