Gastrointestinal duplications: clinical experience

Introduction: Duplications are rare congenital anomalies that can be seen in any part of gastrointestinal tract and whose etiology remains unknown. With this study, it was intended to introduce experiences of a clinic regarding this subject.

Patients and Methods: The patients who were operated because of duplication cysts within a decade were screened retrospectively.

Results: As the result of the screening, twelve patients, five of whom are female and seven are male, were determined. The three patients are newborn infants. The median operation age was 42,5 months (21 days-14,5 years). One of the patients got the diagnosis of intrathoracic duplication, one thoracoabdominal duplication, two gastric duplication, one gastroduodenal duplication, one jejunal duplication, four ileal duplication, one colonic duplication, and one rectal duplication. Through antenatal tests, mass was detected in three patients. Six patients went to doctor because of complaints like abdominal pain, emesis and/or constipation. Two patients got incidental detections. In a patient who got a diagnosis of meconium ileus and underwent an operation for opening ileostomy, ileal atresia and duplication were detected during the closure of this ileostomy. It was considered that ten of the patients had duplication in the differential diagnosis before the operation. The duplication diagnosis for two other patients was made during the operation. Ten of them were cystic duplications and two other were tubular. All the patients had complete surgical excision. It was detected that 3 patterns (for 2 ileal and 1 colonic duplications) examined in the histopathology had ectopic gastric mucosa. One of the duplications in which ectopic tissue detected was perforated. It was not seen any risk for major morbidity or mortality in the series.

Conclusion: The gastrointestinal tract duplications vary greatly in presentation, size, location and symptoms. Despite the fact that small bowel is the most diseased part, duplications can be seen in any part of the gastrointestinal tract as it is seen in the series. In parallel with the development and commonness in the screening methods, it is possible to diagnose many cases before operation or in an early age. There is a possibility that the duplications in which gastric mucosa contains, cause perforation or complications like haemorrhage. These duplications can rarely be seen together with atresias. They can show neoplastic changes in the long term. Although the surgical approach can vary according to the location of the duplication, the essential principles are to preserve healthy tissues as far as possible, to completely resect if possible, and to completely excise the cystic mucosa if not possible. The results are satisfactory in the long term.

Sindirim sistemi duplikasyonları: klinik deneyim

Giriş: Duplikasyonlar sindirim sisteminin herhangi bir seviyesinde görülebilen ve etyolojisi tam bilinmeyen nadir doğumsal anomalilerdir. Bu çalışmada bir kliniğin konu ile ilgili deneyimin sunulması amaçlandı.  

Hastalar ve Yöntem: 10 yıllık bir sürede duplikasyon kisti nedeniyle ameliyat edilen hastalar geriye dönük olarak tarandı.

Bulgular: Taramada 5’i kız, 7’si erkek olan 12 hasta saptandı. Üç hasta yenidoğandı. Ortanca ameliyat yaşı 42,5 aydı (21 gün-14,5 yıl). Hastaların 1’inde intratorasik, 1’inde torakoabdominal, 2’sinde gastrik, 1’inde gastroduodenal, 1’inde jejunal, 4’ünde ileal, 1’inde kolonik ve 1’inde rektal duplikasyon vardı. 3 hastada antenatal tetkiklerde kitle saptanmıştı. 6 hasta karın ağrısı, kusma ve/veya kabızlık yakınmalarıyla başvurdu. 2 hastada tesadüfi tanı kondu. Dış merkezde mekonyum ileus tanısıyla ileostomi açılan bir hastada ise ileostomi kapatılması sırasında ileal atrezi ve duplikasyon saptandı.  Hastaların 10’unda ameliyat öncesinde ayırıcı tanıda duplikasyon düşünülürken, 2’sinde tanı ameliyatta kondu. Duplikasyonların 10’u kistik ve 2’si tubülerdi. Hastaların tümünde tam cerrahi eksizyon yapıldı. Histopatolojide 3 örnekte (2 ileal ve 1 kolonik) ektopik mide mukozası saptandı. Ektopik doku saptanan duplikasyonların 1’i perfore idi. Seride major morbidite ve mortalite görülmedi.

Sonuç: Sindirim sisteminin duplikasyonları , başvuru, boyut, yerleşim ve bulgular açısından büyük varyasyon gösterirler. Her ne kadar en sık tutulan organın ince barsak olduğu bilinirse de seride görüldüğü üzere, tüm seviyelerde yerleşebilirler. Görüntüleme yöntemlerindeki gelişim ve yaygınlaşmaya paralel olarak çoğu olguda ameliyat öncesinde ve erken yaşta tanı koymak mümkündür.  Gastrik mukoza içeren duplikasyonların perforasyon ve kanama gibi komplikasyonlara yol açma olasılığı vardır. Nadiren atrezilerle birlikte görülebilirler. Uzun dönemde neoplastik değişiklikler gösterebilirler. Cerrahi yaklaşım, yerleşim yerine göre farklıklar gösterebilirse de sağlıklı dokuların olabildiğince korunması, mümkünse tam rezeksiyon yapılması, mümkün olmayan hallerde kist mukozasının tamamen çıkartılması temel ilkelerdir. Uzun dönem sonuçlar yüz güldürücüdür.
