Does Neonatal Pneumoperitoneum Should Always Be Considered As An Absolute Indication For Surgery?

AIM: At a newborn with poor general condition, radiological diagnosis of  pneumoperitoneum suggests gastrointestinal (GI) perforation and requires emergency abdominal exploration. We present a newborn referred to our clinic with preliminary diagnosis of GI perforation, to emphasize that surgical intervention is not always necessary in pneumoperitoneum.
MATERIAL AND METHODS: A 3 day old female newborn has been referred to our clinic with the preliminary diagnosis of GI perforation. The patient was taken to emergency surgery after x-rays and blood tests.
FINDINGS: The patient has a history of respiratory distress syndrome which is diagnosed shortly after birth and required high pressure mechanical ventilation. Because of the sudden deterioration of the general condition and abdominal distention, the patient referred to our clinic. There was tense abdominal distention on physical examination; pneumomediastinum and left pneumothorax and pneumoperitoneum on x-rays. The patient was immediately taken to surgery. Plenty of air was evacuated with tube thoracostomy but no perforation was seen on the exploration of the GI system. After the operation, the patient passed away as a result of sudden heart rhythm disruption immediately after surgery. In some articles, it was mentioned, the pneumothorax and pneumomediastinum occurs as a result of high pressure mechanical ventilation. After that the air proceeds by dissecting the sheaths of the mediastinal large vessels to the peritoneal and retroperitoneal cavity. It is also suggested that to full recovery can be accomplished if it is diagnosed with GI barium studies.
RESULTS: Pneumothorax and pneumomediastinum without additional finding which occurs during high pressure mechanical ventilation should be consider spontaneous pneumoperitoneum. In these cases, the patient's recovery can be achieved without the need for surgery. However, at the newborns with low birth weight and poor general condition, emergency surgery is not a big mistake regarding of GI perforation.
 

Yenidoğandaki Pnömoperitoneum Her Zaman Cerrahi Endikasyon Olarak Değerlendirilmeli mi?

AMAÇ: Genel durumu kötü olan bir yenidoğanda radyolojik olarak pnömoperitoneumun görülmesinde ilk tanı gastrointestinal sistem (GİS) perforasyonudur ve acil karın eksplorasyonunu düşündürür. Ancak her zaman pnömoperitoneumda cerrahi girişimin gerekli olmadığını vurgulamak için bu yazıda kliniğimize gastrointestinal perforasyon ön tanısı ile yönlendirilen bir yenidoğan sunulmuştur.
GEREÇ VE YÖNTEM: Dış merkezde 34 yaşındaki bir anneden 31. haftada ikiz eşi olarak 1200 gm. doğan 3 günlük kız yenidoğan GİS perforasyonu ön tanısı ile kliniğimize gönderilmiştir. Genel durumu kötü olan hasta, rutin incelemeler, akciğer ve Ayakta Direkt Batın Grafisi (ADBG) sonrası acil olarak ameliyata alınmıştır.
BULGULAR: Hastamızın öyküsünde doğumdan hemen sonra Respiratuar Distres Sendromu (RDS) tanısı ile mekanik ventilatörle yüksek basınç uygulanarak izlendiği öğrenilmiştir. İzlem sırasında hastanın genel durumunda aniden kötüleşme ve karında şişlik gelişmesi üzerine GİS perforasyonu düşünülerek kliniğimize gönderildiği belirlenmiştir. Fizik bakıda genel durumu kötü olan hastada karında distansiyon; akciğer grafisinde pnömomediastinum ve sol pnömotoraks; ADBG’de yaygın pnömoperitoneum görülmüştür. Hasta acil olarak ameliyata alınmış; ancak tüm GİS’in taranmasında perforasyon saptanamamıştır. Ameliyat sırasında sol hemitoraksa yapılan tüp torakostomiden bol miktarda hava gelişi olduğu belirlenmiştir. Spontan pnömoperitoneum düşünülerek hastanın ameliyatı sonlandırıldıktan hemen sonra ani gelişen kalp ritmi bozulmaları sonucu hasta kaybedilmiştir. Yayınlarda basınçlı mekanik ventilasyon sonucu önce pnömotoraks ve pnömomediastinumun geliştiği; daha sonra da havanın mediastendeki büyük damarların kılıflarını disseke ederek retroperitona ve peritona geçerek pnömoperitoneumu oluşturduğu öğrenilmiştir. Opaklı GİS incelemeleri ile tanı konulursa ameliyat gerekmeden tam iyileşme sağlandığı ileri sürülmektedir.
SONUÇ: Basınçlı mekanik ventilasyon sırasında pnömotoraks, pnömomediastinım sonrası ek bulgu olmadan ve hastanın genel durumunu bozmadan ani gelişen pnömoperitoneumda spontan pnömoperitoneum düşünülmelidir. Bu durumda ameliyat gerekmeden hastanın iyileşmesi sağlanabilir. Ancak düşük doğum kilolu ve genel durumu kötü olan yenidoğanlarda yine de GİS perforasyonu düşünülerek acil ameliyat uygulamak büyük bir hata değildir.
