Although acute appendicitis is one of the most common reason s of acute abdominal surgery, torsion of appendix vermiformis is a very rare condition and often diagnosed intraoperatively.  A five year old boy was admitted to pediatric clinics with nausea and abdominal pain for three days.  Because of the ultrasonographic intussusception findings , the patient consulted with pediatric surgery.  Even though there wasn’t any emergency abdominal surgery symptom like vomiting, fever, guarding, rebound, distantion we decided to operation because of existing palpable mass in the right side of abdomen and ultrasonographic findings.  White cell count was 12000/mm3.

                At the time of surgery we met serohemorrhagic fluid like intussusception and a retrocaecal gangrenous appendix vermiformis rotated 720 degrees clockwise at a point of 1 cm from its base.  Appendectomy was performed and the patient was discharged home on the second postoperative day without any complications.

Akut apandisit sık görülen akut abdominal cerrahi sebeplerinden olmasına rağmen apendiks vermiformis torsiyonu oldukça nadir görülen ve teşhisi sıklıkla operasyon sırasında konulan bir durumdur.  Beş yaşındaki erkek hasta çocuk kliniğine karın ağrısı ve bulantı şikâyeti ile başvurdu.  Batın ultrasonunda 2 cm kolonik invajinasyon ve target sign işareti görülmesi üzerine çocuk cerrahisi konsültasyonu istenen hastada akut cerrahi gerektirecek ateş, defans, rebound, kusma, distansiyon gibi bulgular olmamasına rağmen sağ alt kadranda kitle palpe edilmesi ve USG bulguları ile operasyona karar verildi.  Lökosit sayımı 12000/mm3 idi.  Operasyonda invajinasyondakine benzer serohemorajik mai ile karşılaşıldı.  Kitle eksplore edildiğinde radiksi 720 derece saat yönünde torsiyone olmuş gangrenöz apandisiks ile karşılaşılarak apandektomi yapıldı.  Hasta postop 2. Gün komplikasyonsuz taburcu edildi.
