A Rare Cause of Intestinal Obstruction in Infants; Duplication Cysts

Introduction: Enteric duplication cysts are relatively common congenital anomalies that occur throughout the alimentary tract. DK  may be asymptomatic or present with vague symptoms

mimicking some other common pathologies

Case: 3 day old male case. His physical examination was normal with  signs of gastrointestinal obstruction. He had no signs of  radiographic intestinal obstruction. The baby underwent diagnostic laparotomy that identified öne lesions in the left  lower quadrant:  2x1 cm in size a classic ileal duplication cyst that was bulging out of the wall of the ileum. The  cysts were excised and   ileo-ileal  was performed with a primary anastomosis and was discharged home

Discussion: Duplication cysts are conjenital abnormanilaties that can be place in all  gastrointestinal tractus.They are usualy asemptomatic and may usualy established incidentaly during rutin examinations. It can affect any part of the gastrointestinal tract with ileum being the most common site. In the partial resection of ectopic gastric mucosa, the cyst should be excised since acid secreting mucosa may lead to complications such as haemorrhage, erosion, or malignant degeneration.

Conclusion: Duplications cysts can be a diagnostic quandary to the surgeon because of its varied presentment, a high index of doubt is necessary to facilitate diagnosis, prompt operative intervention after enough resuscitation may be necessary in a setting where diagnostic tests are either not available or affordable.

Nadir Bir İntestinal Obstrüksiyon Nedeni: İleal Duplikasyon Kisti

Giriş: Çocuk cerrahisi kliniklerinde, yenidoğan gastro intestinal sistem (GİS) obstrüksiyonları oldukça komplextir. Nadir  bir intestinal obstrüksiyon nedeni ise dublikasyon kisti (DK)’dir.

Olgu: 3 günlük yenidoğan erkek olgu; doğumunun 2. günü  batın distansiyonu sonrası safralı kusma anamnezi olan hasta intestinal obstrüksiyon ayırıcı tanısı için lavman opak kolon grafisi çekildi. GİS atrezisi açısında kullanılmamış kolon, hirschspruns açısında transizyonel zon veya kistik fibrozis ile ilgili herhangi bir bulgu saptanmadı.  Hasta explore edildi. Terminal ileumda lümene bası yapıp obstrüksiyona sebep olan 2x1 cm ebatlarında 1 adet ileal DK saptandı. Wedge rezeksiyon-anastamoz yapıldı. Postop problemsiz  externe edildi.   

Tartışma: GİS’in DK’leri, etiyolojisi tam olarak bilinmeyen tek veya birden fazla olabilen konjenital anomalileridir (1). Etiyolojisinde fetal barsak divertikülünün devam etmesi, vakuolizasyon, kaudal duplikasyon ve split notokord teorileri ileri sürülmüş ve en çok kabul gören teori split notokord sendromu olmuştur (2). DK normal gastrointestinal sistemin epiteline sahip olmaları ile diğer kistik lezyonlardan ayırt edilebilirler (3). Lokalizasyon yerine göre nadiren obstrüksiyon, kist içine kanama, volvulus, kist torsiyonu, kist rüptürü, kistin enfekte olması, üriner ya da bilier obstrüksiyon, malignite (%3 sarkom, lenfanjiosarkom) gibi komplikasyonlar oluşturabilir (1,2,4).

Sonuç: DK farklı klinik durumlarda karşımıza çıktığından dolayı cerrahın tanı koyması zordur. Kesin tanı koyan bir test olmadığından bazen explorasyonla tanı konmaktadır. DK cerrahisinde ise yeterli rezeksiyon yapılması oldukça önemlisdir.
