A fortunate penetrating chest trauma: isolated thoracic wall stab wound

Patients
with penetrating chest trauma should undergo a rapid and accurate diagnostic
work-up followed by an adequate surgical management in order to keep their
prognosis relatively good. We report a case of a child presenting with stab
wound on her right hypocondrium. A 3-year-old girl was taken to the emergency
department due to penetrating thoracic wall injury. She had suffered an assault
with a knife to her right hypocondrium while playing in the kitchen. The knife
had been put into the dishwashing machine with the sharp tip looking upwards.
By accident the girl had fallen down to the knife and a penetration of the
knife to the right hemithorax was observed. On admission, the patient was well
in normovolemic state with stable vital function. Physical examination revealed
a penetrating wound to the right hypocondrium and lower thoracic wall with a
diameter of 2 cm. Chest roentgenogram revealed neither hemo-pneumothorax nor
signs of pulmonary parenchymal injury. The patient was transferred to the
operating room for wound exploration and appropriate surgical management. A
metal probe was inserted into the wound edge and a subcutaneous tract with a
measurement of 4-cm was found. It was postulated that the position of the knife
with its sharp tip looking upwards made it appear as a subcutaneous traumatic
tract. The intercostal muscles were found to be violated and hopefully the
visceral and parietal pleura was found to be intact without producing any
pulmonary parenchymal injury. Primary suture closure with fine absorbable
materials was performed and the postoperative recovery was uneventful. This
report highlights the unpredictability of knife injury in children with
penetrating thoracic wall trauma. These injuries may easily be missed if
surgical exploration of the wound is not performed.

penetran travmalı olgularda hızlı ve uygun tanısal çalışmayı takiben yeterli
cerrahi girişimle prognoz iyi olabilmektedir. Sağ hipokondrium, sağ hemitoraksa
nafiz kesici-delici alet (mutfak bıçağı) yaralanması ile başvuran bir olgu
kliniğimize kabul edilmiştir. Üç yaşındaki kız olgu bulaşık makinesine sivri ve
keskin ucu yukarı bakacak şekilde konulan bıçak sebebiyle, mutfakta oynarken
sağ hipokondrium bölgesine bıçak yarası almış ve kazaen bıçağın üstüne
düşmesiyle bıçağın sağ hemitoraksa penetrasyonu oluşmuştur. Kliniğe kabulü
esnasında olguda normovolemik durum ve stabil vital fonsiyonlar mevcuttu. Fizik
muayenede sağ hipokondriumda alt toraks duvarında 2 cm çapında penetran göğüs
yaralanması saptandı. Göğüs grafisinde hemo-pnömotoraks veya pulmoner parankimal
incinme bulguları görülmedi. Yapılan cerrahi eksplorasyonda metal prob yara
ağzından içeri sokuldu ve yaklaşık 4 cm. uzunlukta subkutanöz traktüs saptandı.
Anılan subkutanöz incinmenin sivri ve keskin ucu yukarı bakan bıçak tarafından
meydana getirildiği düşünüldü. İnsizyon kraniale doğru yaklaşık 4 cm uzatılarak
yapılan detaylı cerrahi eksplorasyonda şans eseri viseral ve parietal plevranın
salim olduğu, pulmoner parankimal incinmenin bulunmadığı, interkostal kasların
incindiği saptandı. İnce emilebilir materyelle primer sütürasyon yapılan olguda
postoperatif gidişat sorunsuz seyretmiştir. Göğüse penetran travmalı olgularda
hızlı ve uygun tanısal çalışmayı takiben yeterli cerrahi girişimle prognoz iyi
olabilmektedir. Bu çalışmada penetran göğüs travmalı çocuklarda bıçak yarasının
şiddetinin önceden kestirilemeyecek derecede ciddi olabileceği ve uygun cerrahi
eksplorasyon yapılmazsa kolaylıkla gözden kaçırılabileceğinin altı çizilmek
istenmiştir.
