Second layer flaps in the proximal hypospadias repair

Aim: We evaluated the efficacy of the second layer flaps used to prevent the development of the urethrocutaneous fistula after proximal hypospadias repair.

Patients and Methods: Because of the peno-scrotal hypospadias and severe penile chordee, 

36 cases (18 months to 10 years), who previously performed chordee release and Byars flap were brought to the ventral surface of the penis, were admitted for second stage hypospadias repair. In the second stage repair, neourethra was created by tubularized of the plate in the ventral penile surface, and obtained second layer flaps laid onto the neourethra. The second layer flaps were harvested from the tunica vaginalis in 22 patients (group 1), and the dartos flap which was obtained from latero-ventral surface of the penis, was used in 14 patients (group 2) who tunica vaginalis flap could not be taken. The urethral catheter was withdrawn on postoperative days 12 to 15.  

Results: In group 1, subcoronal urethrocutaneous fistula developed in the only one patient (4%), and distal dehiscence (from glans to midpenile area) was detected in three patients (18%), and urethral stenosis response to the dilatations developed in one case (%4). In group 2, of 14 children, three had distal dehiscence (28%).  In regard to the  complications there was no significant difference between both groups (p>0.05).

 Conclusion: Although the flaps could not be shown superiority from each other, in the proximal hypospadias repair, using of these both flaps as second layer is also easy and feasible an alternative method for reduce the incidence of the complications such as urethrocutaneous fistula. 

Proksimal hipospadias onarımında ikinci tabaka flepler

Amaç: Proksimal hipospadias onarımı sonrası üretrokutanöz fistül gelişimini engellemek için kullanılan ikinci tabaka fleplerin etkinliğini değerlendirdik.

Hasta ve Metod: Penoskrotal hipospadias ve şiddetli penile kordi nedeniyle daha önce kordi düzeltilmesi yapılan ve penisin ventraline Byars flebi getirilen 36 olgu (18 ay-10 yaş), ikinci aşama hipospadias onarımı için kabul edildi. İkinci aşama onarımda yeni üretra, ventral penil  yüzeydeki düzlemin tübülarize edilmesiyle oluşturuldu ve elde edilen ikinci tabaka flepler,  yeni üretra üzerine serildi. İkinci tabaka flepler, 22 hastada (grup 1) tunika vajinalisden elde edildi ve penisin latero-ventral yüzünden elde edilen dartos flebi ise, tunika vajinalis febi alınamayan 14 hastada (grup 2) kullanıldı. Üretral kateter, ameliyat sonrası 12.-15. günler arasında çekildi.

Sonuçlar: Grup 1’de sadece bir hastada (%4) subkoronal üretrokutanöz fistül gelişti ve 4 hastada (%18) distal ayrılma (glanstan midpenil alana kadar) tespit edildi ve bir olguda (%8) dilatasyonlara cevap veren üretral stenoz gelişti. Grup 2’de 14 hastanın dördünde (%28)  distal ayrılma vardı. Komplikasyonlar açısından iki grup arasında anlamlı bir farklılık yoktu (p>0.05).

Sonuç: Proksimal hipospadias onarımında fleplerin birbirine herhangi bir üstünlüğü gösterilememesine rağmen,  ikinci tabaka flep kullanımı, üretrokutanöz fistül gibi komplikasyonların sıklığını azaltmak için kolay ve uygulanabilir alternatif bir yöntemdir.
