A giant cystosarcoma phylloides of the mammary gland in an adolescent : A Case Report

Cystosarcoma phylloides of the mammary gland is rarely seen in all age groups, and even much more rarely during adolescency. They are fibroepithelial tumours with a potential for recurrence, and metastatic spread. Its primary treatment is an extensive surgical excision including contours of the normal mammary tissue

 

A 13-year-old Syrian girl consulted to our polyclinic with complaints of rapidly growing painless mass in her left breast for the previous 2 months. Her family history was unremarkable. On physical examination, a solid, mobile compact  mass with regular contours measuring nearly  11x6.5X5 which filled the upper, and lower outer quadrants of the left breast, and caused a prominent asymmetry was found. Physical examination of the contralateral breast  did not reveal any pathology. On breast ultrasonograms (US), a heterogenous hypoechoic superficial  solid mass measuring 10 x 6 cm with lobulated  contours in the left breast was detected. Laboratory data was within normal limits. Under general anesthesia, skin, and subcutaneous incisions were made on the upper , and lower outer  quadrants of the left breast  and the mass with lobulated contours which adhered to the surrounding tissues were totally excised together with the healthy peripheral mammary tissue. Postoperative complication was not encountered, and on histopathological evaluation cellular atypia together with stromal proliferation was consistent with cystosarcoma phyllodes tumour. Any evidence of tumour was not detected on surgical margins, and tumour recurrence was not encountered  up to the 1. year of the postoperative follow-up period.

In the differential diagnosis of the patients admitted with the indication of a breast mass, rarely seen cystosarcome phyllodes should be also taken into consideration  In the treatment an extensive surgical excision encompassing the contours of the healthy mammary tissue , and also close follow-up of the patient regarding the development of recurrences are important issues.

 

 

 

Bir adolesanda memenin dev sistosarkoma filloides tümörü: Olgu Sunumu

Tüm yaş gruplarında nadir görülen memenin sistosarkoma filloides tümörüne adolesan dönemde çok daha az rastlanır. Fibroepitelyal tümörler olup, nüks ve metastatik yayılım potansiyeline sahiptirler. Primer tedavisi, normal meme dokusu sınırlarının sağlandığı geniş cerrahi eksizyondur.

 

On üç yaşındaki Suriyeli kız çocuğu son iki aydan bu yana sol memede ağrısız hızla büyüyen kitle şikayeti ile polikliniğimize başvurdu. Aile özgeçmişinde özellik yoktu. Fizik muayenede, sol memede belirgin asimetriye neden olan üst dış ve alt dış kadranı tamamıyla dolduran yaklaşık olarak 11x6.5X5 cm’lik kıvamı sert, mobil, düzenli sınırlı kitle saptandı. Karşı taraf meme muayenesinde patolojik bulguya rastlanmadı. Yapılan meme ultrasonografi (USG) görüntüsünde sol memede 10x6 cm’lik, yüzeyel yerleşimli, lobüle konturlu, heterojen hipoekoik solid kitle tespit edildi. Laboratuvar tetkikleri normal değerlerde idi. Hasta genel anestezi altında, sol meme üst ve alt dış kadrana yapılan cilt ve cilt altı kesisi ile çevre dokulara yapışık, yüzeyi lobüle konturlu kitle, çevre sağlam meme dokusu ile birlikte total olarak çıkarıldı. Ameliyat sonrası komplikasyon yaşanmayan hastanın histopatolojik değerlendirmede gözlenen atipi ile beraber stromal proliferasyon sistosarkoma filloides tümörü ile uyumlu idi. Cerrahi sınırlarda tümör saptanmayan hastanın ameliyat sonrası 1. yıl takibinde nükse rastlanmadı.

Adolesan döneminde memede kitle nedeniyle kabul edilen hastalarda ayırıcı tanıda nadir görülen filloides tümörü düşünülmelidir. Tedavide normal meme dokusu sınırlarının sağlandığı geniş cerrahi eksizyon yapılması ve nüks gelişimi açısından yakın takip önemlidir.
