Pulmonary Mucoepidermoid Carcinoma and Solid Pseudopapillary Neoplasm of the Pancreas: A Coincidence of Two Rare Tumors in 8-year-old

Aim: The primary pulmonary mucoepidermoid carcinoma (MEC) is extremely rare in children and characterized by mucus-secreting and squamous cells combination. Solid pseudopapillary neoplasm (PPN) of the pancreas, namely Frantz tumor, is a rare tumor of the pancreas which is mostly seen in young women. Solid PPN is characterized by polygonal epithelial cells and cystic spaces leading to pseudopapillary appearance and it is associated with β-catenin gene mutation. Herein, we presented an 8-year old boy previously diagnosed and treated with MEC and incidentally diagnosed PPN in order to draw attention to the possibility of co-existence of these two rare primary tumors.


Case: An 8-year old boy previously diagnosed as MEC. He was treated surgically. He was admitted to our hospital with the incidentally recognized mass at pancreas during postoperative work-out studies. The lower sections of chest computerized tomography revealed a hypodens mass which is 1 × 1 cm in diameter and located at the middle of the pancreas. Distal pancreatectomy was performed and the spleen was spared. Microscopically, the lesion was diagnosed as solid PPN which was an encapsulated tumor consisting of eosinophilic and pleomorphic cells surrounded by spindle shaped cells. Immunohistochemically, tumor cells were positively stained with β-catenin, neuron specific enolase, and CD56.


Conclusion: Two rare tumors presenting at a rare range of age, and resulting in a very rare coincidence have been reported in the present paper. Surgery was curative for both tumors of the presented case. To the best of our knowledge, this is the first reported case of PPN concomitant with MEC in the literature.

Pulmoner Mukoepidermoid Karsinom ve Pankreasın Solid Psödopapiller Neoplazmı: Ender Görülen İki Tümörün 8 Yaşında Çocukta Birlikteliği

Amaç: Primer Pulmoner mukoepidermoid karsinom (MEK) çocuklarda çok ender görülür ve mukus salgılayan hücrelerle yassı hücrelerden oluşur. Pankreasın solid psödopapiller neoplazmı (PPN), diğer adıyla Frantz tümörü, çoğunlukla genç kadınlarda görülen ender bir tümördür. PPN psödopapiller görünüm oluşturan çokgen şekilli epitelyum hücreleri ve kistik alanlardan oluşur. PPN, β-katenin gen mutasyonu ile karakterizedir. Ender görülen bu iki tümörün birlikte görülebilme olasılığına dikkat çekmek amacıyla, daha önce MEK tanısı alıp sağaltılmış ve rastlantısal olarak PPN saptanan 8 yaşında olgu sunulmuştur.


Olgu:  8 yaşında erkek hasta daha önce MEK tanısı alıp, lobektomi yapılarak  sağaltılmış ve  Ameliyat sonrası tarama incelemelerinde rastlantısal olarak saptanan pankreas kitlesi  nedeniyle merkezimize başvurmuştur. Göğüs bilgisayarlı tomografi incelemesinin alt kesitlerinde, pankreas orta kesiminde 1×1 cm çapında hipodens lezyon görülmüştür. Dalak korunarak distal pankreatektomi yapılmıştır. Mikroskopik incelemede solid PPN olarak rapor edilen tümör kapsüllü olup, iğsi hücrelerle çevrilmiş, eozinofilik ve pleomorfik hücrelerden oluşmaktaydı. İmmünohistokimyasal çalışmada tümör hücreleri β-katenin, nöron spesifik enolaz, ve CD56 ile pozitif boyanmıştır.    

  

Sonuç: Çocuk yaş grubunda ender görülen iki tümörün birlikteliği sunulmuştur. Her iki tümör cerrahi eksizyon yapılarak sağaltılmıştır. Bildiğimiz kadarıyla olgumuz, MEK ve PPN tümörlerinin birlikteliğinin ilk ve tek örneğidir.
