Molecular Evaluation of Pediatric Hydatid Cyst in Eastern Turkey: Preliminary Result

Introduction: Cystic echinococcosis is a major public health problem World wide, including Turkey. The different strains of E. granulosus responsible for human hydatidosis. However, the molecular characterization of E. granulosus genotypes of pediatric patients is inadequate in Turkey. The aim of the current study was to evaluate the strain(s) of E. granulosus in operated pediatric cases in eastern Turkey.

Methods: Between years 2012 to 2014, 10 pediatric patients, ages ranging from 3 to 15, with various complaints were included in this study. The cases, whose cyst hydatid hemagglutination tests were positive, were diagnosed with ultrasonography and computerized tomography, and diagnoses were confirmed by the surgery. Cyst fluids and germinative membrane pieces were analysed by PCR technique.

Results: The patients consisted of 7 boys and 3 girls, eight patients had only liver hydatid cyst while two patients had multiple cysts of various organs. There were two ruptured liver cysts. Molecular analysis was performed on hydatid cyst samples obtained from the patients residing in 4 different provinces in eastern Turkey. According to mt-12S rRNA PCR results, all cases were found to be G1/G3 cluster (E. granulosus sensu stricto) which was called “sheep strain”. No evidence of strain G-4 - G-10 in these samples.

Conclusion: We can preliminarily conclude that common sheep strain (G1/G3) may more possible for pediatric cases due to the domestic cycle. 

Türkiye’nin Doğusunda Çocuk Kist Hidatiklerinin Moleküler İncelenmesi: Ön Sonuç

Amaç: Kist hidatik dünyada olduğu gibi Türkiye’de de önemli bir halk sağlığı sorunudur. İnsanlarda farklı E. granulosus suşları hastalıktan sorumludur. Türkiye’de çocuk olgularda E. granulosus genotiplemesi için moleküler inceleme yetersizdir. Bu çalışmanın amacı Türkiye’nin doğusunda kist hidatik ameliyatlı çocuklarda E. granulosus suş(lar)ının belirlenmesidir.

Methods: 2012 – 2014 yılları arasında, yaşları 3-15 arasında değişen 10 çocuğa ait materyalle çalışıldı. Hastalar hemaglütinasyon testleri pozitif, US ve BT ile tanımlanmış ve cerrahi ile tanıları kesinleşmiş olgulardı. Ameliyatta elde edilen kist sıvıları ve germinatif membran parçalarında PCR tekniği ile çalışıldı.

Bulgular: Hastaların 7’si erkek 3’ü kızdı. Sekizinde sadece karaciğer kist hidatiği var iken ikisinde multiorgan tutulumlu kistler vardı. İki karaciğer kisti rüptüreydi. PCR incelemesinde mt-12S rRNA sonuçlarıyla tüm olgularda eskiden koyun suşu olarak adlandırılan G1/G3 suşu (E. granulosus sensu stricto) bulundu. Diğer suşlara ait (G4-G10) bir bulgu elde edilmedi.  

Sonuç: Bu bulgularla çocuklardaki kist hidatiğin evcil hayvan sirkülasyonlu koyun suşundan kaynaklandığı ön sonucuna varabiliriz.
