Proximal Anterior Flep Technique for Esophageal Atresia Surgery

Aim: The mortality of esophageal atresia (EA) with tracheaesophageal fistula (TEF) is lower than 10%; however the anastomosis stricture is still problem in a ratio of 18-51%. We present proximal anterior flep techniqe udeveloped in our clinic.

Materyal-Method: We evaluated our patients retrospectively with age, sex, the lenght of gap, anastomosis stricture,  gastroesophageal  reflux (GEF) and dilatation between 2006-2015. After finding and fastening the TEF; proximal esophagus has cateterised and measured the lenght of the gap between proximal and distal. Proximal esophagus has isolated, sutured, picked up and prepered the  flep by cutting half of the posterior wall circularly. The distal esophagus has interrupted aproximately 5 mm vertically from the anterior wall  and spatulised with 6/0 monofilaman suture. The posterior wall sutured and than the proximal anterior wall flep sutured to the distal esophagus.

Results: We have operated totally 16 patients. A patient has died early postoperative days because of serious cardiac problem and prematurity. The other 15 patient’s birth weeks were between 32 and 38 weeks, 8 of them were boy and 7 of them were girl. The length of the gaps were 3 cm in 4 cases, 2,5 cm in 2 cases, 2 cm in 2 cases, 1,5 cm in 4 cases and 1 cm in cases. All patients has any early anastomosis failure or secondary TEF. Esophagram has done after 4 or 5 months from surgery and has any anastomosis stenosis. We have find GER all the long gap patients. Any dilatation has need the follow up between  8 months and 10 years. Only two patients have need anti-reflux surgery.

Discussion: Tight and circular anastomosis is raising the risc of stricture. We procure oblique and large anastomosis and think that there is any stricture cases by reducing the pressure of anterior wall with the flep. 

Özofagus Atrezisi Cerrahisinde Proksimal Poş Anterior Flep Yöntemi

Amaç: Özofagus atrezisi (ÖA)– Trakeaözofageal fistül (TÖF) mortalitesi %10’un altına inmiş olmasına rağmen anastomoz striktürü %18-51 oranlarında görülmesiyle halen sorun olmaya devam etmektedir. Bu çalışmamızda kliniğimizde geliştirilen proksimal poştan oluşturulan anterior flep yöntemini sunuyoruz.

 

Materyal ve Metod: 2006-2015 yılları arasında yöntemimizi uyguladığımız olgular; yaş, cinsiyet, özofagus uçları arasındaki açıklık, anastomoz striktürü, gastroözofageal reflü (GÖR) ve dilatasyon açısından geriye doğru değerlendirildi.

 

Ameliyat tekniği: Sağ torakotomi ve ekstraplevral yaklaşımla TÖF bulunup bağlandıktan sonra proksimal poş içine yerleştirilen sonda yardımıyla önce iki uç arasındaki açıklık ölçüldü. Proksimal poş ağız tabanına kadar serbestleştirildi. Daha sonra proksimal uca askı suturu konup yukarı kaldırılıp uca yakın arka duvarda yarım dairesel kesi yapılıp flep hazırlandı. Distal özofagusun ön duvarında vertikal yaklaşık 5mm uzunluğunda insizyon yapılıp spatülize edildi. Önce arka duvar tek tek 6/0 eriyebilen monoflaman dikişlerle anastomoz yapıldı. Daha sonra proksimal poş ön duvardaki flep distal özofagusa adapte edilip tek tek aynı dikişle dikildi.

 

Sonuçlar: 2006-2015 yılları arasında 16 ÖA – TÖF hastası kliniğimizde opere edildi. Ciddi kardiyak problemi olan 32 haftalık bir yenidoğan erken post operatif dönemde kaybedildi. Yaşayan diğer 15 olgu değerlendirildi. Olguların yaşları 32-38 hafta arasında değişmekteydi. Olguların 8’i erkek, 7’si kızdı. Özofagus uç aralıkları 4 olguda 3 cm, 2 olguda 2,5 cm, 2 olguda 2 cm, 4 olguda 1,5 cm ve 3 olguda 1cm idi. Hiçbir olgumuzda erken anastomoz kaçağı ya da sekonder TÖF gelişmedi. Striktür ve GÖR açısından tüm olgulara 4-5. aylarında özofagogram yapıldı ve anastomoz darlıkları yoktu. Özellikle iki uç açıklığı fazla olan olguların hepsinde GÖR tespit ettik. 8 ay ile 10 yıl arasında takiplerimizde olguların hiçbirinde dilatasyon ihtiyacı olmadı. İki olgumuzda ilave GÖR için antireflü cerrahisi yapıldı.

 

Tartışma: Gergin ve dairesel anastomoz striktür riskini artırmaktadır. Yöntemimizde oblik şekilde ve geniş bir anastomoz elde edilmektedir. Ayrıca elde edilen flep ön duvardaki anastomoz gerginliğini de azalttığı için olgularımızda striktür gelişmediğini düşünmekteyiz.
