Umbilical Cord Hernia

Aim: Hernia into the umbilical cord is an entity, has been poorly understood and often miscategorized as ‘omphalocele minor’. As opposed to omphaloceles, which are well described anomalies, the literature on umbilical cord hernias is limited. The purpose of this study was to describe the characteristics and outcomes of umbilical cord hernias.

Materials and Methods:  We enrolled 14 cases with umbilical cord hernias for ten years. The patients' data were retrospectively reviewed, and preoperative preparation of the newborn, gestational age, birth weight, other associated malformations, the surgical technique used, enteral nutrition, mechanical ventilation, postoperative scar healing, and length of hospitalization were recorded.

Results: This study includes 14 infants with umbilical cord hernias. The gestational age of these infants ranged from 32 to 40 weeks. Birth weight ranged between 2270 g and 4840 g. Only 6 of 14 cases with umbilical cord hernias were diagnosed by antenatal ultrasound, the rest were unscreened pregnancies. There were four cases with associated malformations, namely agenesis of the right kidney, left kidney with duplex system, coarctation of the aorta, ventricular septal defect, meningocel, club foot and choanal atresia. All cases underwent abdominal ultrasonography. The sizes of the hernias at the time of diagnosis ranged from 20 to 35 mm in twelve of the cases. In two cases, the size of the hernia 40mm. The abdominal wall layers in the two cases were all in their correct anatomical locations, in which the bowel herniates into the base of a normally inserted umbilical cord. Surgery was performed on the first day of life and included immediate exploration of the cord and repositioning of the intestines into the abdomen. Surgical exploration was done in all cases and the neonates underwent corrective surgery with good outcomes. 2 patients with major congenital anomalies died due to sepsis, respiratory distress, interstitial pneumonia postoperative 2nd and 3th days. Patients were discharged between 6 to 8 days after surgery.

Conclusions: In an umbilical cord hernia, the body folds develop normally and form the umbilical ring. Surgical outcome is excellent due to lack of association with other congenital.

Amaç: Umblikal kord hernisi az anlaşılmış ve sık sık 'omfalosel minör "olarak yanlış kategorize edilmiş anomalidir. Iyi tarif edilmiş olan omfalosel anomalilerinin aksine umblikal kord hernisi ilgili literatür sınırlıdır. Bu çalışmanın amacı, Umblikal kord hernisinin karakteristik özelliklerini ve sonuçlarını açıklamak.

Gereç ve Yöntemler: On yıllık sürede umblikal kord herni tanısı alan 14 olguyu çalışmamıza dahil ettik. Olguların verileri retrospektif olarak incelendi ve yenidoğanın ameliyat öncesi hazırlığı, gebelik yaşı, doğum ağırlığı, diğer ilgili malformasyonlar, kullanılan cerrahi teknik, enteral beslenme, mekanik ventilasyon, postoperatif skar iyileşmesi ve hastanede kalış süresi kaydedildi.

Bulgular: Bu çalışmaya umblikal kord hernili 14 olgu dahil edildi. Olguların gebelik yaşı 32 ile 40 hafta arasında değişmektedir. Doğum ağırlığı 2270-4840 g (median:2700g) arasındaydı. Umblikal kord hernili olguların sadece 6’sı antenatal ultrasonografi ile teşhis edildi, gerikalan takipsiz gebelikler idi. Dört olguda sağ böbrek agenezisi, sol böbrekte çift toplayıcı sistem, aort koarktasyonu, ventriküler septal defekt, meningosel, çarpık ayak ve koanal atrezi eşlik eden malformasyonlardı. Tüm olgulara abdominal ultrasonografi yapıldı. Olguların 12’sinde herninin boyutları 20-35mm arasında değişmekteydi. İki olguda ise herninin boyutu yaklaşık 40mm’di. Bu iki olguda karın duvarı katmanları, tabanına bağırsak herniasyou olan göbek kordonu doğru anatomik yerleşimliydi. Tüm olgulara yaşamlarının ilk gününde eksplorasyon ve iyi sonuçları olan düzeltici cerrahi uygulandı. İki olgumuz öldü.  Major ek anomalileri olan iki hasta postoperetif  2. Ve 3. günlerde sepsis, interstisyel pnömoni nedeniyle öldü. Olgular 6-8 gün arasında hastaneden taburcu oldu.

Sonuç: Umblikal kord hernisinde vücut kıvrımlarının ve göbek halkasının gelişimi normaldir. Eşlik eden anomali olmadığında cerrahi sonuçlar tamin edici düzeydedir.
