Pygopagus Conjoined Twins

Aim: Conjoined twinning is one of the most uncommon congenital abnormalities. The birth of conjoined twins is a very difficult and dramatic moment for parents. Diagnose and treatment in this extraordinary situation requires close cooperation of the multidisciplinary medical team. We report a case of pygopagus conjoined twins.

Case:  Male conjoined twins were delivered by cesarean section at 36 weeks’ gestation at a peripheral hospital. They were referred to our neonatal intensive care unit after postnatal 2th day. They had a combined weight of 5.2 kg, each weighing about 2.6 kg.  Joined back to back at the coccyx region was detected by the physical examination. Each had a separate anus that passed meconium. Assessment of the central nervous system, heart, and lungs revealed no abnormalities. Plain radiograph of the spine showed the sacral to be separate, but fused coccyx without any other vertebral anomalies. Magnetic resonance imaging of the spine showed that the spinal canal was ending at the level of the first lumbar vertebra. There wasn’t any other sacral vertebral anomaly.  It was exhibited that each of the coccyx angled to outward and fused with the other like a tail.

The preoperative assessment was performed by pediatric surgery, neurosurgery, orthopedy and anesthesia clinics together. All surgical and anesthesia equipments which will be used at separation prepared as double for each patient. Fused coccyx was removed at the level of the sacrococcygeal region in each twin by 2 hours taken operation. After separation, the skin was closed using V-shaped skin flaps primarily. Oral feeding was started on the 2th postoperative day. They were discharged home on the 14th postoperative day, and on followed during 10 months postoperativly. They were healthy and thriving normally without any residual problem.

Conclusions: A detailed anatomic study of the twins and surgical planning must precede separation. The diagnostics and treatment in this extraordinary situation requires close cooperation of the multidisciplinary medical team.

Amaç: Yapışık ikizler en nadir görülen konjenital anomalilerden biridir. Yapışık ikizlerin doğumu, aileler için çok zor ve dramatik bir andır. Bu olağanüstü durumun teşhis ve tedavisi multidisipliner sağlık ekibinin yakın işbirliğini gerektirir. Bir yapışık ikiz olgusunu sunduk.

Olgu: Erkek yapışık ikizler, gestasyonun 36. haftasında özel bir tıp merkezinde sezaryen ile dünyaya geldi. Doğumdan 2 gün sonra hastanemiz yenidoğan yoğun bakım ünitesine kabul edildi. İkizlerin her biri yaklaşık 2,6 kg, toplam 5,2 kg ağırlığındaydı. Fizik muayenelerinde sırt sırta dönük pozisyonda ve koksigial bölgeden yapışık oldukları görüldü. Her birinin ayrı anüsü vardı ve  mekonyum çıkışı normaldi. Santral sinir sistemi, kalp ve akciğerlerin değerlendirilmesin de ek anomaliye rastlanmadı. Direkgrafide koksikslerinde füzyon dışında başka vertebral anomalileri yoktu.  Manyetik rezonans görüntülemede birinci lomber vertebra seviyesinde spinal kanalın sonlandığını görüldü. Tüm omurgada sakrumlarda dahil anomali yoktu. Her bir koksiks dışa doğru açı yaparak, bir kuyruk gibi diğeri ile kaynaşmış durumdaydı. Cerrahi öncesi değerlendirme çocuk cerrahisi, beyin cerrahisi, ortopedi ve anestezi bölümleri ile birlikte yapıldı. Ayırma işlemi yapılacak olan ameliyat odasında tüm cerrahi ve anestezi ekipmanları her bir hastaya ayrı olarak hazırlandı. Her bir ikizin sakrokoksigeal seviyeden füzyone olan koksiksleri 2 saat süren bir operasyonla çıkarıldı. Ayırma işleminden sonra cilt defekti V şeklinde cilt flep ile primer kapatıldı. Postoperatif 2. gün genel durumları iyi olan hastalara beslenme başlandı. Operasyondan 14 gün sonra taburcu edildiler. 10 ay süren ameliyat sonrası takiplerinde sağlıklı oldukları tespit edildi.

Sonuç: İkizlerin detaylı anatomik çalışması ve planlama, cerrahi ayırmadan önce gelmelidir. Bu olağanüstü durumun teşhis ve tedavisi multidisipliner sağlık ekibinin yakın işbirliğini gerektirir.
