Simultaneous bilateral primary spontaneous pneumothorax

Simultaneous bilateral primary spontaneous pneumothorax (PSP) is a very rarely seen condition generally in male smokers as a result of rupture of subpeural blebs.

A-12-year-old asthenic girl was admitted because of sudden onset of right-sided chest pain, and shortness of breath. Her parents were smokers. On physical examination her pulse rate was 110/min, and respiratory rate 26/min. Percussion of her right hemithorax revealed hyperresonance, and decrease in respiratory sounds. On standing PA chest radiogram,  a line  in the right hemithorax representing visceral pleura, and increase in the density of collapsed lung were seen. Tube thoracostomy resolved pneumothorax. Seven days later PA chest X-ray revealed bilateral pneumothorax which resulted in simultaneous placement of thoracostomy tubes in both hemithoraxes  Bleb formation was not encountered on thoracal CT. During 2 years of follow-up after the second tube thoracostomy, any recurrence did not develop

Primary treatment of PSP is tube thoracostomy. However, a complete consensus has not been reached concerning treatment approach in pediatric patients presenting with recurrent PSP. In adult patients video-aided thoracoscopy or open thoracotomy followed by pleurectomy or pleurodesis is recommended.  

We think that tube thoracostomy will generally suffice for the treatment of the first recurrence of PSP in the pediatric age group.

Eşzamanlı bilateral primer spontan pnömotoraks

Eşzamanlı bilateral primer spontan pnömotoraks (PSP), genellikle genç sigara içen erkeklerde subplevral bleblerin patlaması sonucunda ortaya çıkan son derece nadir bir durumdur. Oniki yaşındaki astenik yapılı bir kız olgu, günlük aktivite sırasında, ani başlayan sağ taraf göğüs ağrısı ve nefes darlığı nedeni ile kabul edildi. Anamnezinde ebeveynlerinde sigara içme öyküsü mevcuttu. Fizik muayenede, nabız 110/dk olup, solunum sayısı 26/dk idi. Sağ hemitoraksta perküsyonda hiperrezonans ve solunum seslerinde azalma mevcuttu. Ayakta çekilmiş PA akciğer grafisinde sağ hemitoraksta viseral plevraya ait çizgi ve kollabe akciğerin dansitesinde artma görülmekte idi. Yapılan tüp torakostomi ile pnömotoraks düzeldi. Yedi gün sonra PA akciğer grafisinde bilateral pnömotoraks tespit edilmesi üzerine, eş zamanlı olarak  her iki hemitoraksa ayrı ayrı tüp torakostomi uygulandı. Toraks bilgisayarlı tomografisinde blebe rastlanmadı. İkinci kez tüp torakostominin 2 yıllık takibinde nüks gelişmedi.

PSP'nin primer tedavisi tüp torakostomidir. Ancak nüks ile gelen çocuk yaş grubunun tedavi yaklaşımında hala tam bir görüş birliği sağlanamamıştır. Bu durumda,erişkinlerde video destekli torakoskopi ya da açık torakotomi ile plörektomi ya da plöredezi yapılması önerilmektedir.

Küçük yaş grubunda ise, birinci PSP nüksünde tüp torakostominin tedavi için genellikle yeterli olabileceğini düşünüyoruz.
