İntroduction:
İsolated blunt abdominal trauma caused by jejunal perforation is rare and most
often seen after motor vehicle accidents. After non-vehicle traffic
accident was presented to isolated cases of jejunal perforation.
Case: Twelve-year-old
male patient was admitted to our hospital after car crash. There was
minimal abdominal distention and tenderness on physical examination. The
patient was admitted to our clinic. There was no free air image in the X-ray.
Contrast-enhanced abdominal CT showed that the patients taken free
intraperitoneal air. The exploratory laparotomy, 1 piece, 30 cm distal
to the ligament of Treitz, 2x1 cm in size were determined, jejunal perforation.
Resection anastomosis was made. Post op 5. day were fed. Patients
who tolerate and was discharged post op 12. day. Conducted a successful
outcome with clinical suspicion and timely exploration was obtained.
Conclusion: Although
rare, blunt abdominal trauma, intestinal perforation should be kept in mind
that the arrival of isolation. Contrast-enhanced abdomen CT should be
avoided.

Giriş: Künt abdominal travmanın neden
olduğu izole jejunal perforasyon nadirdir ve çoğu kez motorlu taşıt kazalarından
sonra görülür. Bildiride araç dışı trafik kazası (ADTK) sonrası izole jejunal
perforasyon olgusu sunuldu.
Olgu: Oniki yaşında erkek hasta ADTK
sonrası hastanemize getirildi. Fizik muayenesinde batın hassasiyeti ve minimal
distansiyonu vardı. Hasta kliniğimize yatırıldı. Grafilerinde serbest hava
görüntüsü yoktu. Kontrastlı batın BT çekilen hastada serbest intraperitoneal
hava bulunduğunu görüldü. Tanısal laparotomide , treitz ligamanının 30 cm
distalinde yer alan 2x1 cm boyutlu jejunal perforasyon belirlendi. Rezeksiyon
anastomoz yapıldı. Post op 5. Günde beslendi. Tolere eden hasta post op 12.
günde taburcu edildi. Klinik kuşku ve zamanında gerçekleştirilen eksplorasyonla
başarılı bir sonuç elde edildi.
Sonuç: Künt abdominal travma sonrası nadir
de olsa, izole intestinal perforasyonların gelişebileceği akıldan
çıkarılmamalıdır.
