Laparoscopic hepatico-duodenostomy for an atypical choledochal cyst

Aim: To present a case with atypical choledochal cyst that performed hepatico-duodenostomy with laparoscopic approach in this video.

Case: A 20 month-old female presented with complaints of intermittent crampy upper abdominal pain and vomiting for a month. There was no past history of fever and jaundice. Physical examination did not reveal any significant abnormality. Labarotary investigations including liver function tests were within normal limits. Ultrasound examination showed a cystic hepatic lesion. Then the patient underwent magnetic resonance cholangiopancreaticography (MRCP) which revealed that there was a cystic dilatation in choledochus and a cystic dilatation of left hepatic duct (Todani, Type 4B ?). There was no associated anomalous pancreatico-biliary junction.

Three  5 mm port were used and ecartation of liver was achieved by suturing the ligamentum falciforme hepatis to the abdominal wall. Then gall bladder, cystic choledochus and cystic left hepatic duct were excised and distal end of choledochus was ligated with 4/0 vicryl suture and divided. After that, left and right hepatic duct were separetaly anastomosed to the duodenum with interrupted 5/0 vicryl sutures. A suction drain was inserted to the peritoneal cavity.

Result: The operation time was about 180 minutes. Feeding was started in 3rd postoperative day. Easly postoperative period was uneventful. The patient was discharged from the ward at 5th postoperative day.

Conclusion: The firsts choice operation for choledochal cyst is hepatico-jejunostomy Roux-n-Y. Hepatico-duodenostomy is an other alternative teqnique for treatment of choledochal cyst and number of related papers is increasing with minimally invasive ages.

Atipik bir koledok kistinin tedavisinde laparoskopik hepatiko-duodenostomi

Amaç: Bu videoda, atipik bir koledok kistinde yapılan laparoskopik hepatiko-duodenostomi işlemini sunmayı amaçladık.

Olgu: Yirmi aylık kız hasta, kramp tarzında karın ağrısı ve kusma şikayetiyle başvurdu. Hastanın hikayesinde sarılık ya da ateş şikayeti yoktu. Fizik muayenede belirgin bir patoloji saptanmadı. Karaciğer fonksiyon testleri normal sınırlarda idi. Ultrasonografide karaciğerde kistik bir oluşum izlendi. MR kolanjiyopankreatikografide (MRCP) koledok ve sol hepatik kanalda kistik genişleme tespit edildi (Tip 4B ?, Todani sınıflaması). Pankreatiko-biliyer bileşkede ek anomali saptanmadı.

Laparoskopide 3 adet 5mm’lik port kullanıldı. Karaciğer ekartasyonu falsiform ligamandan geçilen sütur ile sağlandı. Safra kesesi, koledok kisti ve kistik sol hepatik kanal eksize edildi. Koledok distali 4/0 vicryl ile bağlandı ve eksize edildi. Sol ve sağ hepatik kanallar, 5/0 vicryl ile, duodenuma ayrı ayrı anostomoz yapıldı. Batına bir adet hemovak dren yerleştirildi.

Bulgular: Operasyon süresi yaklaşık 180 dakika idi. Ameliyat sonrası erken dönemde komplikasyon olmadı. Hasta ameliyat sonrası 3. gün oral beslenmeye başlandı ve 5. gün taburcu edildi. İki ay sonra yapılan kontrol muayenesinde patoloji tespit edilmedi.

Sonuç: Koledok kistlerinin tedavisinde ilk seçenek hepatikojejunostomi ve Roux-n-Y işlemidir. Minimal invaziv cerrahinin yaygınlaşmasıyla, son zamanlarda, koledok kistlerinin tedavisinde hepatikoduodenostomi yöntemi ile ilgili yayınların sayısı giderek artmaktadır.
