hypospadiasis repair and liposuction for the patient with severe hypospadiasis and obesity

Aim: We aimed to present a patient with penoscrotal hypospadiasis and buried penis who had penile shaft revealed by “liposuction” method.

Case : Ten years old patient had penoscrotal hypospadiasis repairment gradually in our clinic. The obese patient had excess pubic fat. He had recurrent chordee and buried penis.

Surgical technique:  We performed degloving, penopubic sling, plication,chordee release and Z plasty of penis . Afterward 30cc solution of tumescence was infiltrated and liposuction were performed with 3mm Coleman canule from the right and left suprapubic areas by criss-cross method. Penile shaft was revealed and skin greft from lower abdominal region was sutured to penile shaft. Patient had no postoperative problem.

Discussion: Pubic liposuction is rarely preferred for the treatment of buried penis. There are numerous surgical methods for revealing the penile shaft. Most of the surgeons prefer complete degloving and excision of the fibrous dartos. Fixation of dartos to the Buck fascia at the base of penis, internal anchoring of dermis at the border of dartos excision (Mitrofanoff technique) and restoration of penopubic and penoscrotal angles, releasing of suspansory ligament are some of these methods. Excess fat in the pubic area is a disadvantage for the success. If the result is not satisfactory for the patients with penoscrotal hypospadiais repairmen, liposuction may be an alternative method.

Results: If the patient with severe hypospadisis had excess pubic fat, liposuction can be preferred for better cosmesis. We suggest that lipectomy or liposuction can be performed besides standart methods.

Amaç : Penoskrotal hipospadiaslı gömülü penisi olan hastada pubik “liposuction” yardımı ile penil şaftı ortaya çıkardığımız hastayı sunmayı amaçladık.

Olgu sunumu: Penoskrotal hipospadiası olan 10 yaşındaki hasta, kliniğimizde kademeli olarak onarılmıştı. Obez hastanın pubik yağlanması fazlaydı. Nüks kordisi ve gömülü penisi vardı (Resim 1).

Ameliyat tekniği: Hastaya degloving, penopubik askı, plikasyon, chordee release ve Z plasti işlemleri yapıldı (Resim 2). Ardından 30 cc Tümesans solüsyonu infiltrasyonunu takiben 3 mm’lik künt uçlu Coleman™ kanül ile sağ ve sol suprapubik bölgeden girilerek criss-cross metodla liposuction yapıldı (Resim 3). Penil şaft uzunluğu ortaya çıkarıldıktan sonra alt abdominal bölgeden alınan cilt grefti penil şafta sütüre edildi (Resim 4). Postoperatif takibinde problem gözlenmedi (Resim 5).

Tartışma : Pubik liposuction gömülü penis tedavisinde nadiren tercih edilen yardımcı tekniktir. Penil şafta yönelik çok sayıda tanımlanmış cerrahi tedaviler mevcuttur. Çoğu yazar komplet degloving ve fibrotik dartos eksizyonunu tercih eder. Penis tabanında dartosun Buck fasyasına fiksasyonu, dartos fasyasının eksizyon sınırında dermisin iç yönde ankorlanması (Mitrofanoff tekniği) ile penopubik ve penoskrotal açının restorasyonu, suspansory ligament serbestleme gibi yöntemler kullanılmaktadır (1). Pubik yağlanma bu cerrahilerdeki başarıyı gölgelemektedir. Ağır hipospadiaslı hastalarda üretral mea anatomik lokalizasyona getirilmesine rağmen tatmin edici görüntü sağlanamadıysa, klasik yöntemlere  pubik liposuction eklenebilir (1-5).

Sonuç: Ağır hipospadiaslı hastalarda aşırı pubik yağlanma varsa kozmetik olarak daha iyi sonuçlar sağlamak için liposuction bir seçenek olabilir. Bu gibi durumlarda lipektomi ya da liposuction yöntemlerinin standart tekniklerle kombine edilmesinin uygun olacağını düşünüyoruz.
