Germ cell tumors (GCT) are rare, presenting only 1-3% of childhood tumors. Twenty per cent of GCT are malignant. Anterior mediastinum is a common location of malignant germ cell tumors, but pericardial involvement have been rarely reported. We report a case of a 13-year-old boy who presented with anterior mediastinal GCT involving pericardium. After chemotheraphy on his whole body PET-CT revealed residual mediastinal mass. On right thoracotomy solid mass the to the middle lobe of the right lung and pericardium was detected. The mass is extipated with sharp and blunt dissections from the pericardium and the lung. Mediastinal GCT are rare tumors and their neighbourhood to the vital organs has importance.
