Loss of Small Intestines with Meconium Cyst Formation: Possible An Early Intrauterine Volvulus

Introduction: Intrauterine intestinal volvulus(IUIV) is a very rare life-threatening condition that is difficult to diagnosis prenatally.The outcome depends on the length of viable intestine,level of obstruction,the presence of meconium peritonitis,associated anomalies,birth weight and gestational age.

Case: A girl who was born prematurely at 32 week, as 2320 gr. Antenatal ultrasonography (USG) at 24th week shown intestinal dilatation with hyperechoic intestinal walls. The patient was discussed at council with prediagnoses of intestinal atresia, hirschsprung disease or volvulus. As there was no finding of fetal distress and no clear diagnosis, waiting till 32nd week of gestation was decided by considering the risk of pulmonary insufficiency. The control prenatal USG at 28th week of gestation revealed only intestinal dilatation.

During the spontaneous birth at 32nd week of gestation, newborn was intubated due to pulmonary insufficiency. At physical examination there was soft distention with normal color at abdomen. A nasogastric tube was placed which drained bilious content. Plain abdominal graph revealed air-fluid levels. The ultrasonography reported as “fluid distention at stomach and intestines, dilated small intestines without abscess”. Contrast colon graph revealed unused colon.

At 5th day of life laparotomy was done. During operation, cystic, stiff, yellow colored, fibrinous pouch was opened which later thought as a meconium cyst. Colon was intact but the ileum and distal jejunum was absent. There was only 25 cm jejunum was present and jejunostomy and cecostomy were done. As postoperative enteral feeding discharged undigested from jejunostomy we concluded presence of short bowel syndrome and started TPN.  At 40th day the patient was lost because of TPN-related sepsis.

Discussion: Is presence of hyperechogenicity at bowel wall mean decreased perfusion and should lead to an early intervention? Could an early intervention of fetal surgery at 24th week of gestation for IUIV have saved the patient? How could be sure about loss of perfusion to face up the risks of fetal surgery?

At our patient vanishing of small intestine segments may be due to IUIV, internal herniation or occlusion of mesenteric vessels. Presence of meconium cyst also points presence of perforation and inflammation. The IUIV that occurred at earlier weeks of gestation might led to intestinal atresia or meconium peritonitis. We believe that this pathology is a result of an early IUIV.

Prenatal ultrasonographic findings of IUIV reported as polihydroamniosis, dilated intestinal loops, abdominal mass, ascites, peritoneal calcifications, increased abdominal circumference, fetal distress and decreased fetal heart rate which mostly seen after 27th week. Unfortunately, these findings are non-specific and due to lack of typical signs definitive diagnosis is difficult.  Definitive diagnosis only made by presence of “whirlpool or snail configuration” of intestines at prenatal ultrasonography which is very hard to define. Although our case has had dilated intestinal loops since 24th week of gestation there was neither other signs of volvulus nor signs of fetal distress indicating a serious pathology. Presence of dilated intestines without any other pathology may be a normal variant.  Only presence of hyperechogenity at bowel walls made us suspicion of decreased perfusion. But lack of definitive diagnosis drawback us from early intervention.

Although most cases of IUIV did not present short-bowel syndrome, this case might be due to early occurrence (24th week) of volvulus in utero. Presence of meconium peritonitis led us to ostomy instead of anastomosis. Unfortunately, TPN dependent nutrition ended with sepsis and the loss of the patient.

Conclusion:  The presence of hyperechogenity of bowel walls accompanied by dilated bowel segment at fetal ultrasonography may point pathologies leading bowel ischemia, such as IUIV which may need urgent fetal surgery. 

Muhtemel Intrauterin Volvulus: Intrauterin Barsak Kaybı ve Mekonyum Kisti Oluşumu

Giriş:Intrauterin intestinal volvulus(IUIV) çok nadir,hayatı tehdit eden ve prenatal tanısı çok zor bir sorundur.Hayatta kalma sağlam barsağın uzunluğuna,tıkanıklığın seviyesine,mekonyum peritonit varlığına,eşlik eden anomalilere,doğum ağırlığına ve gestasyonel yaşa göre değişir.

Olgu:Prematür(32 hft), 2320 gr doğan hastanın;24.gestasyon haftasında(GH) yapılan ultrasonda(USG) barsak dilatasyonu-hiperekoik barsak duvarları görüldü. Konseyde, intestinal atrezi-hirshprung hastalığı-volvulus öntanılarıyla tartışıldı.Fetal stress bulgusu olmaması,akciğer yetmezliği riski göz önüne alınarak 32. GH’ye kadar beklenmesi kararlaştırıldı.Kontrol USG’sinde(28. GH) sadece barsak dilatasyonu tespit edildi.Doğumda solunum yetmezliği nedeniyle hasta entübe edildi.Muayenesinde abdomen normal renkte, yumuşak distandü.Nasogastrikden safralı gelenleri oldu.Ayakta karın grafisinde hava-sıvı seviyeleri,USG “mide ve barsak segmentlerinde sıvı distansiyonu-ince barsaklarda dilatasyon”mevcut,abse görülmedi.Kolon grafisinde kullanılmamış kolon görünümü vardı.Beşinci gününde laparotomi yapıldı.Ameliyatta mekonyum kisti olduğu anlaşılan,kistik,fibrinli,sert,sarı,poşun içine girildi.Kolon intakttı,ileum ve jejunumun bir kısmı yoktu.Sadece 25 cm jejunum mevcuttu,jejunostomi ve çekostomi açıldı.Oral aldığı besinlerin jejunostomiden sindirilmeden gelmesi üzerine,hastaya kısa barsak tanısı ile parenteral beslenme(TPN) başlandı.Hasta 40. gün TPN’ye bağlı sepsis nedeniyle kaybedildi.

Tartışma:Barsak duvarında hiperekojenite olması azalmış perfüzyona işaret eder mi? Erken girişim gerektirir mi? Hastamızda 24. GH’den önce fetal cerrahi ile IUIV için erken girişim yapılması hayat kurtarıcı olur muydu? Perfüzyon kaybından, fetal cerrahi riskleri göğüsleyecek kadar nasıl emin olabiliriz?

Hastamızda ince barsakların kaybolması IUIV,internal herniasyon-mesenterik tıkanıklığa bağlı olabilir. Mekonyum kistinin varlığı perforasyon-inflamasyon geçirdiğini işaret etmektedir.Erken GH’de görülen IUIV intestinal atrezi ve/veya mekonyum peritonitine yol açabilir.Hastamızdaki patolojinin IUIV olduğunu düşünmekteyiz.IUIV’nin prenatal bulguları polihidramniyos,dilate barsak lupları,abdominal kitle,asit,peritoneal kalsifikasyonlar,artmış karın çevresi,fetal distress,düşük fetal kalp atımı çoğunlukla 27. GH’den önce görülmektedir.Ne yazık ki,bu bulgular non-spesifiktir ve tipik bulgu olmadığından tanı konulması güçtür.Kesin tanı sadece prenatal ultrasonda bağırsaklarda,görülmesi zor olan ‘girdap veya salyangoz işareti’nin görülmesi ile konur.Hastamızda 24. GH’dan itibaren dilate bağırsaklar görülse de,ne volvulusu düşündüren başka bir bulgu ne de fetal distres bulgusu vardı.Başka bir patoloji olmadan dilate bağırsakların olması normalin varyantı kabul edilmektedir.Sadece bağırsak duvarlarında ekojenite artışı azalmış perfüzyonu düşündürtmüştür.Kesin tanının konamaması bizi erken girişimden uzaklaştırmıştır.IUIV vakalarının çoğunda kısa barsak sendromu görülmezken,bu vakada volvulusun erken (24. GH) olmasıyla olmuş olabilir.Mekonyum peritonitin olması bizi anastamoz yerine ostomi yapmaya yöneltmiştir.TPN beslenmesi sonucunda sepsis gelişmiş ve hasta kaybedilmiştir.

Sonuç:Fetal ultrasonda dilate bağırsak segmentlerinin eşlik ettiği hiperekojen bağırsak duvarları varlığı IUIV gibi iskemiye giden bir patolojiyi işaret edebilir ve acil cerrahi girişim gerektirebilir.
