Iatrogenic Tracheal Laceration during Bronchoscopy Which Was Treated without Surgical Intervention

Introduction: One of the most important causes of death during childhood is foreign body aspiration. The treatment is achieved by rigid bronchoscopy which presents many complications, like laryngeal edema, hemorrhage, atelectasis, pneumothorax, pneumomediatinum and death.  We would like to discuss our case with iatrogenic tracheal rupture (laceration) (ITR) which was treated conservatively.

Case: A 19-month-old girl admitted into hospital with cough and wheezing after having fall down while having an almond at her mouth. At physical examination, oxygen saturation was 97%, respiratory sounds was diminished at right lung. At direct graph there was hyperaeration at right lung. At rigid bronchoscope, foreign body was seen at right main bronchus. Foreign body which was grasped by forceps, stuck at level of vocal cords so that we broke it into pieces and removed pieces by both forceps and suction. One piece moved to the segmental bronchi of right lower lobe and stuck there. We could not take it out although we tried for ten minutes. At that time patient showed two episodes of cough reflex, at which broncoscope was withdrawn into right main bronchus and waited for the anesthesia to be deepened. At last, we gave up and decided to redo broncoscopy later.

While bronchoscope was being taken out, we recognized 0,5 cm longitudinal mucosal laceration at right corner of membranous trachea, just superior to tracheal bifurcation. Broncoscopy ended immediately. At first while breathing spontaneously, the patient had transient decreases at oxygen saturation that needed positive ventilation with mask. As crepitation appeared at neck we decided to intubate the patient. At direct graph there was left pneumothorax, which was drained by tube placement. At control graph, left pneumothorax was reduced, there was pneumomediastinum. The patient was taken to intensive care unit and broad-spectrum antibiotic treatment was started. On the first day, patient extubated herself. As her spontaneous respiration was adequate, the patient followed with nasal oxygen. Arterial blood gas controls were within normal limits. Air leak from chest drainage ended. At 5th day chest tube removed, at 8th day she was discharged. The control examinations were normal. We recommend second look after recovery but the patient missed the follow up appointment.

Discussion: Mostly, ITR is a complication of intubation with an incidence of 0.05-0.37%. Also ITR during rigid bronchoscopy has been reported previously at adult cases. Subcutaneous and/or mediastinal emphysema, dyspnea, pneumothorax, haemoptysis should alert surgeon about ITR which have a mortality rate of 22%. Typically ITR are longitudinal lacerations at distal posterior trachea, mostly at right side of pars membranosa. Treatment of ITR is under debate recently. While early surgical repair is accepted traditionally, evidence that supports conservative treatment is increasing recently especially at pediatric cases. Conservative treatment is suggested at clinically stable patient that have non-progressive symptoms with superficial lacerations less than 2cm length. Conservative treatment contains endotracheal intubation, pleural drainage if necessary and broad-spectrum antibiotic treatment. Our patient well tolerated spontaneous respiration and as laceration (rupture) was 0,5 cm length and superficial (mucosal), the drainage of pneumothorax and broad-spectrum antibiotic treatment under close monitorization was sufficient for treatment and no surgery was needed.

Conclusion: During dealing with a stuck foreign body, complications of broncoscopy should be kept in mind. Conservative treatment can be tried at ITR cases according to clinical situation of patient and length and depth of the laceration. 

Bronkoskopi Sırasında İatrogenik Trakea Laserasyonu ve Cerrahi Müdahale Olmadan Tedavisi

Giriş: Çocukluk çağındaki ölümlerin önemlilerinden biri de yabancı cisim aspirasyonudur. Tedavi, larinks ödemi, kanama, atelektazi, pnömotoraks ve ölüm gibi birçok komplikasyona sahip rijid bronkosopi ile yapılabilmektedir. İatrogenik trakea rüptürü (laserasyonu) (İTR) olan ve konservatif tedavi edilen hastamızı tartışmak istiyoruz.

Olgu: On dokuz aylık kız hasta badem yerken düşme sonrası öksürük, hırıltılı solunumu ile başvurdu. Muayenesinde oksijen saturasyonu %97’ydi, akciğer sesleri dinlemekle sağda azalmıştı. Akciğer grafisinde sağda havalanma artışı görüldü. Yapılan rijit bronkoskopide sağ ana bronşta yabancı cisim görüldü. Yabancı cisim forsepsle çıkartılırken vokal korda takıldı. Sonrasında parçalanıp aspire edilerek çoğu çıkartıldı. Bir parçanın sağ alt bronş dallarından birinde olduğu görüldü. On dakika uğraşılmasına rağmen parça çıkartılamadı. Bu sırada hasta hasta iki kere öksürük refleksi gösterdi, bronkoskop sağ ana bronşa geri çekilip anestezinin derinleşmesi beklendi. Sonunda daha sonra tekrar bakılmak üzere işleme son verildi.

Bronkoskop çıkartılırken trakea bifurkasyonun yukarısında, membranöz trakeanın sağ köşesinde, uzunlamasına, mukozal, 0,5cm uzunluğunda laserasyon görüldü. Bronkoskop hemen çıkartıldı. Başlarda spontan solumasına rağmen hastanın aralıklı oksijen saturasyonu düşmesi ile maske ventilasyon gerekliliği oldu. Boyunda krepitasyon olmasıyla hasta entübe edildi. Acil akciğer grafisi sonucu sol pnömotoraks nedeniyle göğüs tüpü takıldı. Kontrolde pnömotoraksın gerilediği, pnömomediastinum olduğu görüldü. Hasta entübe olarak yoğun bakıma çıkartıldı, geniş spektrumlu antibiyotik tedavisi başlandı. Postoperatif 1. gün hasta kendini ekstübe etti. Spontan solunum yeterli olduğu için nasal oksijen ile takip edildi. Kontrol arteriyel kan gazları normaldi. Göğüs tüpünden gelen hava miktarının kesildiği görüldü. Postoperatif 5. gün göğüs tüpü çekildi, 8. gün taburcu edildi. Hastanın kontrolleri normaldi. Hastaya kontrol bronkoskopi önerildi, aile bronkoskopi randevusuna gelmedi.

Tartışma: Çoğunlukla ITR bir %0.05-0.37 oranında entübasyon komplikasyonudur. Bronkoskopi sırasında ITR ayrıca erişkin hastalarda da rapor edilmiştir. Subkutan ve/veya mediastinal amfizem, dispne, pnömotoraks, hemoptizi varlığı cerrahi ölüm oranı %22 olan ITR konusunda uyarmalıdır . Tipik olarak ITR distal posterior trakeada, çoğunlukla membranöz kısımda yerleşimli uzunlamasına laserasyonlardır. ITR tedavisi tartışmalıdır. Geleneksel olarak erken cerrahi tedavi kabul edilirken, son dönemde özellikle çocuklarda konservatif tedaviyi destekleyen kanıtlar artmaktadır. Konservatif tedavi, semptomları ilerlemeyen, laserasyonu 2cm’den kısa ve yüzeyel olan stabil hastalarda önerilmektedir. Konservatif tedavi, endotrakeal entübasyon, gerektiğinde plevral drenaj ve geniş spektrumlu antibiyotik tedavisinden oluşmaktadır. Hastamız spontan solunumu tolere edebilmiştir ve laserasyon (rüptür) 0,5 cm uzunlukta, yüzeysel olduğundan hastamız yakın monitorizasyon ile pnömotoraks drenajı ve geniş spektrumlu antibyotik tedavisi yeterli olmuş ve cerrahi girişime gerek kalmamıştır.

Sonuç: Takıl kalmış yabancı cisimler ile uğraşırken, bronkoskopinin komplike olabileceği unutulmalıdır. ITR’de hastanın klinik durumu ve laserasyonun uzunluğu ve derinliğine göre konservatif tedavi uygulanabilir.
