Approach to ovarian problems in children

Introduction:

Ovarian problems in children are rare and are mostly benign. In our study, patients undergoing surgery for ovarian masses at our clinic were evaluated retrospectively.

Patients and Method:

The records of 255 patients operated for ovarian cyst/mass between 1987-2016 were examined. 146 (57.2%) of the patients, whose age range was 10 days-22 years, were found to have to have simple cysts, 56 (21%) ovarian torsion, and 50 (19.6%) were found to have tumours. 62 (42%) of the ovarian cysts were on the left, 53 (36%) on the right, and 11 (7.5%) bilateral.  24 (49%) of the ovarian torsion cases were left side, 26 (51%) right side, 34 (68%) of tumours right side, 16 (34%) were left side.

The presenting symptoms of the patients were; detection in prenatal period presenting for follow-up, abdominal pain, abdominal distension, vomiting, precocious puberty. Pelvic US was applied to all patients, blood build up was assessed by Doppler US in those with acute abdominal symptoms, tumour markers (AFP, BHCG, CEA, CA-125, CA-19-9) were sent for those with cysts and tumours detected.  In cases with suspected malignant masses, pelvic CT or MRI was taken. The surgical plan (laparotomy / laparoscopy) was made according to the structure, dimensions of the lesion and symptoms.

Findings:

Of the 74 (29%) patients under the age of one with an average mass dimension of 4.9cm, 6 of which detected to have prenatal overian torsion, 5 overian necrosis resulting from strangulated inguinal hernia, 33 were treated with cystectomy, 18 salpingooopherectomy, and 23 with oophorectomy.

Of 97 (38%) cases between the ages of 1-12, with an average mass dimension of 6.4cm, ovarian tumour was detected in 18, torsion in 17 and simple cyst in 62 cases, 15 were treated by salpingooopherectomy, 20 oophorectomy, and 62 cystectomy.

Of 82 (52%) cases over the age of 12, with an average mass dimension of 6.6 cm, 10 were treated with salpingooopherectomy, 12 oopherectomy, and 58 cystectomy. 4 of those with a diagnosis of torsion were detortioned and the ovary was not removed. In torsion surgery, preventive fixation by suture was always carried out on the controlateral ovary.

Conclusion: 

The goal in surgical disease of the ovary is the protection of ovarian tissue where possible. While this is unlikely in the case of prenatal torsion and malignant tumours, in cysts in particular, tissue protective removal even if recognizable ovarian tissue is not visible, and in postnatal torsion, even if the blood supply is suspicious, leaving the ovary in place and if necessary, controlling with a second future procedure, are approaches which must be kept in mind in order to achieve this goal.

 

Çocuklarda over sorunlarına yaklaşım

Giriş:

Çocuklarda over sorunları nadir görülür ve çoğu selimdir. Çalışmada kliniğimizde over kitlesi  nedeniyle ameliyat edilen hastalar geriye dönük olarak değerlendirilmiştir.

Hastalar ve Yöntem:

1987 -2016 arasında over kist/kitlesi nedeniyle ameliyat edilen 255 olgunun kayıtları incelendi. Yaş aralığı 10 gün-22 yaş olan hastaların;  146’sında (% 57,2) basit kist, 56’sında (% 21) over torsiyonu ve 50’sinde (% 19,6) tümör saptandı. Over kistlerinin 62’si (% 42) sol, 53’ü (% 36) sağ ve 11’i (% 7,5) iki taraflıydı.  Over torsiyonlarının 24’ü (% 49) sol , 26’sı (% 51) sağ, tümörlerin ise 34’ü (% 68) sağ, 16’sı (% 34) sol taraftaydı.

Hastaların başlıca geliş yakınmaları; doğum öncesi dönemde saptanıp izlem amaçlı başvuru, karın ağrısı, karın şişliği, kusma, erken ergenlik şeklindeydi. Tümünde pelvis US çekildi,  akut karın bulguları bulunanlarda  Doppler US ile kanlanma değerlendirildi, kist ve kitle saptananlarda tümör belirteçleri (AFP, BHCG, CEA,CA-125, CA-19-9 ) gönderildi.  Habis kitleden  şüphelenilen olgularda ek olarak  pelvik tomografi veya MR çekildi. Cerrahi planı (laparatomi/laparoskopi) ise lezyonun  yapısına, boyutuna ve semptomlarına göre yapıldı.

Bulgular:

Kitle boyutları ortalama 4,9 cm olan, 6’sında doğum öncesinde  over torsiyonu, 5’inde boğulmuş  kasık fıtığına  bağlı over nekrozu saptanan 1 yaş altındaki  74 (% 29) olgunun 33’ünde kistektomi, 18’inde salpingoooforektomi, 23’ünde ooforektomi yapıldı.

Kitle boyutları ortalama  6,4 cm olan 1-12 yaş arasındaki 97 olgunun (% 38) 18’inde over tümörü, 17’sinde torsiyon ve 62’sinde basit kist saptanarak, 15’inde salpingoooforektomi,, 20’sinde ooforektomi ve 62’sinde kistektomi yapıldı.

Kitle boyutları ortalama 6,6 cm olan, 12 yaş üzerindeki 84 (% 52) olgunun 10’una salpingoooforektomi, 12’sine ooforektomi ve 58’ine kistektomi yapıldı. Torsiyon tanılı olanların 4’ünde detorsiyon yapıldı ve over alınmadı. Torsiyon ameliyatlarında daima karşı taraf overin koruma amaçlı dikişle sabitlenmesi yapıldı.

Sonuç:  

Overin cerrahi hastalıklarında  amaç over dokusunun olabildiğince korunmasıdır özellikle kistlerde VE torsiyonlarda over dokusu görünmese yada kanlanmadığı düşünülsede yerinde bırakılmalı, gerekirse ikincil bir girişimle kontrol edilmelidir.
