Monocytes as a new further laboratory markers of Gastroesophageal Reflux Disease

Aim: Gastroesophageal reflux disease (GERD) is defined as the leakage of stomach contents into the esophagus. The diagnosis of GERD esophagogastroduodenoscopy (EGD) or 24-hour pH monitoring (pH 24) is placed on the test. In our study,we investigated whether or not the relationship between reflux disease and laboratory parameters.

Method: In this study, previously diagnosed with acid reflux or esophagitis and medical treatment of patients with ongoing complaints after made GERD questionarre (GerdQ) according to positive (score> 8) in the 6-18 age from 74 children were included. Two physicians who worked on the study. Endoscopic findings, 24-hour pH measurements, body mass index (BMI), vitamin B12 and laboratory findings (monocytes, MCV, MPV, RDW, eosinophils) were investigated.

Results: Patients mean age was 13,82±2,57.  Male/Female: ¼.  While forty two   patients were performed EGD,  thirty two  patients were performed only  24-hour pH test. Ten cases who was  made EGD was found esophagitis. EGD is performed in 28 cases are available in various degrees of gastritis. In 4 cases it did not reveal any pathology. 14 cases were detected acid reflux, which in the 24-hour pH test group. When all cases are examined in the laboratory findings in reflux esophagitis and acid reflux positive cases, monocytes (%) showed a statistically significant increase in value (P<0,01). In cases with  esophagitis and acid reflux ; MCV, MPV, RDW, eosinophilia, B12,BMI were not statistically significant differences.

Conclusion: According to the findings from our study, On the follow-up of treatment of GERD control EGD  and 24-hour pH test which was invasive methods for children suggested that inflammatory monocytes which change the value of the taken we believe that there is a parameter to be considered in the selection of patients. Also we think it is necessary new parameters for monitoring severity and treatment of the disease.

Gastroözofageal Reflü Hastalığında Yeni Bir Labarotuvar Belirteci Olarak Monosit

Amaç: Gastroözofageal reflü hastalığı (GÖRH) mide içeriğinin özofagusa kaçağı olarak tanımlanır. GÖRH’ının tanısı özofagogastroduodenoskopi (ÖGD) veya 24 saatlik pH monitorizasyonu (24 pH) testi ile konulur. Çalışmamızda laboratuvar parametreleri ve reflü hastalığı arasında ilişki olup olmadığını araştırmayı amaçladık.

Gereç ve Yöntem: Bu çalışmaya daha önce asid reflü veya ösofajit tanısı almış  ve medikal tedavi yapılmış sonrasında şikayetleri devam eden olgulardan GÖR anketine (GerdQ) gore pozitif (skor > 8) bulunan 6-18 yaş arası 74 çocuk dahil edildi. İki klinisyen bu çalışma üzerinde çalıştı. Endoskopik bulgular, 24 saat pH ölçümü, vücut kitle indeksi (BMI), vit B12 ve laboratuvar bulguları (Monosit, MCV, MPV, RDW, eozinofili) incelendi.

 Bulgular: Olguların yaş ortlaması 13,82 ± 2,57 idi. E/K: ¼ idi. 42 olguya ÖGD yapılırken 32 olguya ise 24 saat pH testi yapıldı. ÖGD yapılan 10 olguda  özofajit tespit edildi. ÖGD yapılan 28 hastada ise çeşitli derecede gastrit mevcuttu. 4 olguda ise herhangi bir patoloji saptanmadı. 24 saatlik  pH testi grubunda ise 14 olguda asid reflü saptandı.Tüm olguların laboratuvar bulguları incelendiğinde reflü özofajit ve reflü pozitif olgularda Monosit (%) değerlerinde istatistiksel olarak anlamlı bir artış saptandı (P <0.01). Özofajit ve asid reflü saptanan olgularda MCV, MPV, RDW, eozinofili, B12, BMI değerlerinde ise  istatistiksel olarak anlamlı bir fark tespit edilmedi.

 Sonuç: Çalışmamız sonuçlarına göre, GÖRH ‘nın tedavi izleminde, çocuklar için invaziv yöntemler olan; kontrol ÖGD ve 24 saat pH testi yapılma önerisi için, inflamatuvar bir belirteç olan Monosit değerindeki  değişimlerin, hastaların seçiminde dikkate alınması gereken bir parametre olduğunu  düşünmekteyiz. Ayrıca GÖRH’ nın şiddetini beliryeci ve tedavi izleminde yeni labarotuvar parametrelerine ihtiyaç duyulmaktadır.
