Ten years experince in duodenal obstruction

Aim: Our aim is to share data and our clinical experience for patients who were operated in our clinic.

Material ve metod: 16 cases with a diagnosis of duodenal obstruction operated in our clinic between 2006-2016 years. Patients analyzed in terms of gestational age, sex, birth weight, application day, anomalies, operation time, done surgery, feeding time, day of hospitalization and follow up retrospectively.

Results: Nine of the cases were boy and seven were girl. The mean gestational age of cases was 34,8 ( 29-40 ) weeks, birth weights was 2346 ( 3600-1540 ) gr. Cases were referred to our clinic due to the double bubble findings to be seen in abdominal radiography because of 11 cases with vomiting, four cases with abdominal distention and impairment of general condition in one case. The mean of admission age was 2.12 ( (1-8 ) days. Anomalies were observed as congenital heart disease in eight cases, Down syndrome in five cases , and malrotation in two cases. The mean of operation time was 1.3 (1-9) days. Surgical methods were Diamond -Shape duodenoduodenostomy in 14 cases and duodenojejunostomy in two cases due to malrotation . Type 1 atresia in seven cases, annular pancreas in five cases , type 2 atresia in two cases, and ' windsock ' deformity in two cases were seen in operations.The mean of TPN duration of patients 8.4 ( 6-14 ) days , the time to onset of oral feeding 6.31 ( 5-9 ) days ,receiving of and mean duration of hospitalization 17:56 (12-25 ) days. Surgical complications were not observed none of cases and all cases were discharged .

Discussion: Congenital duodenal obstruction is one of the most important cause of intestinal obstruction in the neonatal period and anomalies seen in more than 50% of these cases. In the literature, cardiac anomaly is in 30% of, Down's syndrome is in 30-40% of, gastrointestinal anomalies is in 25% of duodenal obstruction have been reported. In our study, we found that cardiac anomaly is in 50% of cases, Down syndrome is in %31.25 of cases, malrotation is in % 12.5of cases. The incidence of cardiac anomalies is higher than the literature, and the incidence of other anomalies is similar to literature.Post-operative mortality rate has been reported %3-4 in the literature. None of our cases died. We believe that, our intensive care facilities and avoiding to tpn related complications caused this situation.

Conclusion: More careful screening cardiac anomalies, preoperative and postoperative early period follow-up is to be done in the intensive care and the implementation of tpn support in the desired conditions in duodenal obstruction of newborns are important in terms of morbidity and mortality.

Duodenal obstruksiyonlu olgularda 10 yıllık deneyim

Amaç: Kliniğimizde duodenal obstrüksiyon tanısı ile opere edilen olgulara ait verileri ve klinik deneyimlerimizi paylaşmaktır.

Materyal ve metod: 2006-2016 yılları arasında kliniğimizde 16 olgu duodenal obstrüksiyon tanısı ile opere edildi. Hastalar; gestasyonel yaşı, cinsiyeti, doğum ağrılığı, başvuru günü, şikayeti, ek anomalileri, ameliyat zamanı, yapılan ameliyat, beslenme zamanı, yatış süresi ve takip açısından retrospektif olarak incelendi.

Bulgular: Olguların dokuzu erkek, yedisi kızdı. Ortalama gestasyonel yaşları 34.8(29-40) hafta, doğum ağırılıkları 2346 (3600-1540) gr idi. 11 olgu kusma, 4 olgu batın distansiyonu ve bir olguda genel durum bozukluğu nedeniyle çekilen direk grafide double bubble bulgusu görülmesi üzerine kliniğimize sevk edilmişti. Hastaların başvuru yaşı ortalama 2.12 ( (1-8) gündü. Ek anomali olarak; sekiz olguda konjenital kalp hastalığı, beş olguda Down Sendromu ve iki olguda malrotasyon tesbit edildi. Operasyon zamanı ortalama 1,3 (1-9) gündü. Cerrahi yöntem olarak 14 olguya Diamond-Shape Duodenoduodenostomi, iki olguya ise malrotasyon nedeni ile duodenojejunostomi yapıldı. Operasyonlarda yedi olguda tip 1 atrezi, beş olguda anüler pankreas, iki olguda tip 2 atrezi, iki olguda ise ‘windsock’ deformitesi görüldü. Hastalarda ortalama TPN alma süresi 8,4 (6-14) gün, oral beslenmeye başlama süresi 6.31(5-9) gün ve ortalama yatış süreleri 17.56 (12-25) gündü. Hiçbir olguda cerrahi bir komplikasyon görülmedi ve tüm olgular şifa ile taburcu edildi.

Tartışma: Konjenital duodenal obstrüksiyonlar yenidoğan dönemindeki intestinal obstruksiyonların en önemli nedenlerinden biridir ve bu olguların %50 den fazlasında ek anomali görülür. Literatürde %30 kardiyak anomali, %30-40 down sendromu, %25 gastrointestinal anomali bildirilmştir. Bizim çalışmamızda ise olguların %50’sinde kardiyak anomaliği, %31.25’de down sendromu, % 12,5’inde malrotasyon saptadık. Kardiyak anomali sıklığımız literatüre göre yüksek, diğer ek anomalilerin sıklığı ise literatür ile benzerdir. Literatürde post operatif mortalite oranı %3-4 olarak bildirilmiştir. Bizim hastalarımızdan hiç biri kaybedilmemiştir. Bu duruma yoğun bakım imkanlarımızın ve TPN’ye bağlı komplikasyon yaşamamamızın katkı sağladığını düşünüyoruz.

Sonuç: Yenidoğanlarda görülen duodenal obstrüksiyonların kardiyak ek anomali açısından daha dikkatli taranması, preoperatif ve postoperatif erken dönem takibin yoğun bakım şartlarında yapılması ve TPN desteğinin istenilen standartlarda uygulanması morbidite ve mortalite açısından önemlidir.
