Laparoscopic Surgery for Gonadal Interventions in Children with Intersex Disorders

Aim of the study: A gonadectomy, or gonadal biopsy, is indicated in children with intersex disorders. Conventional open gonadal interventions have the same disadvantages as a laparotomy, including prolonged hospital stays and abdominal scarring. This study evaluates the efficacy of laparoscopic surgery on gonadal interventions in children with intersex disorders.Methods: Eight children with intersex disorders underwent chromosomal analysis, were diagnosed, and were treated using laparoscopic gonadal surgery between February 2015 and June 2016 at our clinic. Main results: The chromosomal evaluation indicated that all the patents were 46 XY. The patients’ mean age was 11.85 ± 7.5 years (1–17 years). The patients’ diagnoses were as follows: congenital lipoid adrenal hyperplasia in two patients, androgen receptor defects in three patients, leydig cell hypoplasia in one patient, ovotesticular sexual disorder in one patient, and mixed gonadal dysgenesis in one patient. Seven patients received laparoscopic gonadectomies, but one of these seven patients received a bilateral laparoscopic gonadal biopsy before undergoing gonadectomy. The final patient received a bilateral laparoscopic gonadal biopsy. No postoperative complications were found. Conclusion: Laparoscopic gonadal surgery is an effective and safe procedure for children with intersex disorders. The procedure has the same advantages as laparoscopic surgery, including short recovery periods and superior cosmesis. Laparoscopic surgery also allows for clear views of the area and easy access to the gonads, which facilitates the proper evaluation of internal sexual organs.
