Intestinal perforation due to mucormycossis: A rare intity in pediatric surgery

Introduction: Mucormycosis is invasive fungal infection that often seen in immunocompromised patients. Intestinal mucormycosis is extremely rare, difficult to diagnose, and has dismal prognosis.  

Case: An 8-year-old male patient diagnosed with Burkitt’s lymphoma, thrombocytopenia, febrile neutropenia and common variable immune deficiency was presented. Firstly, he was followed for partial intestinal obstruction for two days. Then he was operated when abdominal tenderness was increased and complete obstruction was occurred. There was double covered perforation of proximal jejunum and ileocecal valve region. Segmental resection and anastomosis was done for proximal jejunum. Ileocecal region was removed then ileostomy and colostomy were done. There was approximately 90cm suspicious vascular compromised terminal ileal segment. Second look surgery was done after 48 hours and reaching up to 120cm of terminal ileum was removed because of intestinal ischemia and vascular thrombosis. Next day, antifungal treatment was given when the histopathology showed intestinal mucormycosis, transmural necrosis and vascular thrombosis. He survived with surgery and appropriate antifungal therapy urgently. Ileostomy and colostomy were closed 6 months later.

Conclusion: Mucormycosis is a fungal disease that may invade the gastrointestinal tract of immunosuppressed, neutropenic patients under chemotherapy and steroid treatment. This infection results in high morbidity and mortality. Early surgery, appropriate antifungal therapy and rising neutrophils are essential for survival. Our patient had subsequently developed a complete intestinal obstruction-acute surgical abdomen secondary to covered perforation resulted from intestinal mucormycosis. He survived with closed follow up, timely surgery and appropriate antifungal treatment policy.

Mukormikozise Bağlı İntestinal Perforasyon: Çocuk Cerahisinde Nadir Bir Durum

Giriş: Mukormikozis sıklıkla immünsüpresse hastalarda görülen invaziv fungal efeksiyondur. İntestinal mukormikozis oldukça nadir, tanısı zor ve prognozu kasvetli olan bir klinik tablodur.   

Olgu: Burkitt lenfoma tanılı trombositopeni, febril nötropeni, yaygın değişken immun yetmezliği olan 8 yaşında bir erkek hasta 2 gün parsiyel intestinal obstruksiyon nedeniyle izlendi. Karın hassasiyetinin artması ve tam obstruksiyon gelişmesi nedeniyle hasta ameliyata alındı. Proksimal jejunumda ve ileoçekal valvde perforasyonu olduğu görüldü. Jejunuma segmental rezeksiyon ve anastomoz uygulandı. İleoçekal valv bölgesi eksize edildi. İleostomi ve kolostomi yapıldı. Terminal ileumda yaklaşık 90cm’lik dolaşımı şüpheli bir segment vardı. 48 saat sonra yapılan ikincil laparatomide 120cm’ye ulaşan bu segment iskemi ve vasküler tromboz nedeniyle eksize edildi. Histopatolojik incelemede intestinal mukormikozis, tam kat nekroz ve vasküler tromboz görülmesi üzerine hastaya antifungal tedavi başlandı. Hasta erken dönemde cerrahi sağaltım, uygun antifungal tedavi ile hayatta kaldı.  İleostomi ve kolostomisi 6 hafta sonra kapatıldı.

Sonuç: Mukormikozis immumsüprese, steroid ve kemoterapi alan nötropenik hastalarda gastrointestinal sistemi tutabilen yüksek morbidite ve mortlite ile sonuçlanan bir mantar enfeksiyonudur. Hastanın hayatta kalabilmesi için erken cerrahi, uygun antifungal tedavi ve nötrofil sayısının yükseltilmesi gereklidir. Hastamızda intestinal mukormikozise sekonder oluşan kapalı intestinal perforasyon sonucu tam obstrüksiyon şeklinde akut cerrahi batın tablosu gelişmiştir. Ancak hastamız yakın takip, zamanında cerrahi sağaltım ve uygun antifungal tedaviden oluşan yönetim ile şifa ile taburcu edilmiştir.
